摘要
目的探讨血浆Fibulin-1蛋白在2型糖尿病合并颈动脉病变中的检测意义及与相关代谢指标的关系。方法收集100例糖尿病患者,其中40例为单纯糖尿病患者,60例为糖尿病合并颈动脉病变者,同期收集40例健康体检者为正常对照组,检测受试对象血浆中Fibulin-1及内皮素(ET-1)、血管内皮生长因子(VEGF)水平及相关生化指标。结果与正常对照组比较,单纯糖尿病组体质指数(BMI)、甘油三酯(TG)、胆固醇(TC)、糖化血红蛋白(HbA1c)、VEGF、ET-1、Fibulin-1及收缩压(SBP)增高(P<0.05),舒张压(DBP)差异无统计学意义(P>0.05);与单纯糖尿病组比较,合并颈动脉病变组BMI、病程、TG、TC、VEGF、ET-1、Fibulin-1及SBP增高(P<0.05),而HbA1c及舒张压(DBP)差异无统计学意义(P>0.05);在校正了颈动脉内膜中层厚度(IMT)相关的其他因素后,ET-1、Fibulin-1、HbA1c升高是IMT增厚的危险因素(P<0.05);Fibulin-1浓度与年龄、病程、SBP、DBP、BMI、HbA1c、TG、TC、ET-1、VEGF均呈正相关(P<0.05)。结论 HbA1c、ET-1及Fibulin-1共同参与了2型糖尿病颈动脉及大血管病变的发生和发展,Fibulin-1可能成为预测2型糖尿病患者大血管病变的一项重要指标。
Objective To explore the significance of serum protein Fibulin-1 level in Type 2 diabetes carotid disease and the relationship between Fibulin-1 and related metabolic index. Methods 40 healthy controls and 100 cases with Type 2 diabetes mellitus were included in this study. The patients were then divided into Type 2 diabetes mellitus group(40 patients)and Type 2 diabetes carotid disease group(60 patients),serum levels of Fibulin-1,ET-1,VEGF and related biochemical indexes of each group were detected. Results The serum level of BMI,TG,TC,HbA1c,VEGF,ET-1,Fibulin-1 and SBP was significantly elevated in Type 2 diabetes mellitus group,compared to healthy controls(P〈0.05),but the level of DBP had no statistical difference.Compared to the Type 2 diabetes carotid disease group,the level of BMI,TG,TC,VEGF,ET-1,Fibulin-1 and SBP in Type 2 diabetes macroangiopathy group had a significant statistical difference,but the level of HbA1c and DBP had no statistical difference. There was significant statistical difference in the level of BMI,TG,TC,VEGF,Fibulin-1and ET-1 among the three groups. After adjusting for various factors of IMT,the increased level of ET-1,Fibulin-1 and HbA1c was the risk factors of IMT thickening. Fibulin-1 were positively correlated with age,duration,BMI,SBP,DBP,fasting blood glucose,TG,TC,glycosylated hemoglobin,ET-1 and VEGF.Conclusion HbA1c,ET-1 and Fibulin-1 participates in the occurrence and development of T2DM carotid artery and vascular lesions. The increase of plasma Fibulin-1 in patients with type 2 diabetes may be an important 1,predictor of vascular lesions to T2DM.
