大型医疗机构连续静脉采血人员手卫生策略的探讨 被引量:17

Hand hygiene strategies for continuous blood collection staff in large medical institutions

摘要 目的通过评价3种手套与4种消毒剂组合的个体效应,为大型医疗机构连续静脉采血人员在工作量大和时间不足情况下提供手卫生策略。方法以若干门诊静脉采血护士为观察对象,在对患者进行连续静脉采血操作中,用快速手消剂对手套进行消毒,监测手套表面的菌落数,比较分析手套/消毒剂的十二种组合类型,每种组合采样30份(共采样360份),并通过手套是否泄漏(共收集手套1392只)、消毒时间和手套物理性状的改变来综合比较分析。结果从消毒效果看,360份样本总体合格率达97.5%(合格样本351份),十二种组合的消毒效果差异无统计学意义;1392只手套中231只发生泄漏,手套泄露率差异有统计学意义(P<0.05),主要与3种手套间材质的不同有关(P<0.01),且随着连续操作次数的增加,手套的泄露呈上升趋势,其差异有统计学意义(P<0.05),手套的泄露部位主要位于拇指与食指指尖(54.5%)。结论在连续采血过程中,快速手消剂进行手套外消毒是安全有效、合理可行的;各医疗机构可结合自身情况,从安全、经济、省时等角度综合考虑,选择合适的手套/消毒剂组合。 OBJECTIVE To compare the individual effects of the combinations of three kinds of gloves and four kinds of disinfectants so as to provide the hand hygiene strategies for the continuous blood collection staff who have heavy workload and lack of time in large medical institutions.METHODS A number of venous blood collection staff of outpatient department were recruited as the study objects.During the continuous venous blood collection,the gloves were disinfected by using rapid hand disinfectants,the bacterial colony counts on the surfaces of gloves were monitored,the 12 types of combinations of gloves/disinfectants were compared,30 samples were collected for each combination(360 samples in total),and the leakage of gloves(totally 1392 gloves were collected),time of disinfection and changes of physical properties of gloves were comprehensively analyzed.RESULTS In the view of the disinfection effect,the total qualified rate of the 360 samples was 97.5% (351qualified samples),and there was no significant difference in the disinfection effect among the 12 combinations.Totally 231 of 1392gloves leaked,and there was significant difference in the leakage rate of gloves(P〈0.05),the leakage rate was associated with the material texture of the three kinds of gloves(P〈0.01),the leakage rate of gloves showed an upward trend with the increase of number of times of continuous operation(P〈0.05);the thump and forefinger fingertips sites were major leakage sites of the gloves,accounting for 54.5% .CONCLUSIONThe disinfection of gloves with rapid hand disinfects is safe,effective and feasible.It is necessary for the medical institutions to choose appropriate gloves/disinfectant combination by overall consideration of the safety,economy and time-saving.
