摘要
目的分析甲基强的松龙联合鞘内注射甲氨蝶呤和地塞米松对狼疮脑病患者脑脊液(CSF)水平的影响。方法采用随机数字表法将51例确诊狼疮脑病患者分组,对照组25例给予甲基强的松龙和鞘内注射甲氨蝶呤治疗,观察组26例给予甲基强的松龙、鞘内注射甲氨蝶呤和地塞米松治疗,对比2组临床疗效和CSF水平改善效果。结果观察组治疗后总有效率92.31%,略高于对照组的80.00%(P>0.05);2组治疗方案均能够降低CSF水平,但观察组降低幅度明显优于对照组(P<0.05);观察组不良反应发生率15.38%,略高于对照组的12.00%(P>0.05)。结论甲基强的松龙冲击治疗联合鞘内注射甲氨蝶呤和地塞米松能够显著改善狼疮脑病患者的预后,降低CSF水平,安全可靠,值得临床推广使用。
Objective To analyze the effect of methylprednisolone combined with intrathecal injection of methotrexate and dexamethasone on the CSF level in the lupus encephalopathy patients.Methods Fifty-one cases of lupus encephalopathy patients were randomly divided into contrast group（n=25）and observation group（n=26）.Patients in the contrast group were given intravenous injection of methylprednisolone as well as intrathecal injection of methotrexate treatment,while patients in the observation group were given intravenous injection of methylprednisolone as well as intrathecal injection of methotrexate and dexamethasone treatment.The clinical curative effects and the improvement effects of CSF level were compared.Results After treatment,the total effective rate in the observation group（92.31%）was slightly higher than the contrast group（80.00%,P 0.05）.Both treatment schemes can decrease the CSF level after treatment.Meanwhile,the effect in the observation group was superior to the contrast group（P0.05）.The adverse reaction occurrence rate in the observation group（15.38%）was slightly higher than that of the contrast group（12.00%,P0.05）.Conclusion Methylprednisolone combined with intrathecal injection of methotrexate and dexamethasone can significantly improve the prognosis of lupus encephalopathy,decrease the CSF level safely and reliably.Thus,it deserves to be widely promoted in clinical field.
