摘要
目的探讨经超声内镜(EUS)、CT对慢性胰腺炎的诊断价值。方法选取2015年8月至2016年11月北京市昌平区中医医院消化内科收治的疑似慢性胰腺炎患者216例,均经EUS、CT检查,并以细针穿刺活检诊断结果为金标准。观察比较两种检测方法的诊断价值。结果 216例患者经穿刺活检诊断136例为慢性胰腺炎,80例非慢性胰腺炎。以临床穿刺活检的确诊结果为金标准,EUS灵敏度98.53%,特异度96.25%,准确度98.53%;CT灵敏度93.38%,特异度86.25%,准确度93.38%。EUS检查:24例胰管扩张型胰腺炎,42例肿块型胰腺炎,40例肿块+胆管扩张型胰腺炎,28例弥漫肿大型胰腺炎。CT检查:41例肿块型胰腺炎,39例肿块+胆管扩张型胰腺炎,26例弥漫肿大型胰腺炎,21例胰管扩张型胰腺炎,胰管扩张,胰腺结石/钙化。结论 EUS诊断慢性胰腺炎的灵敏度、特异度高,可清晰观察到患者胰腺实质结构和胰管的细小改变。
Objective To discuss the diagnostic value of endoscopic ultrasonography （EUS） and CT for chronic pan- creatitis. Methods From Aug. 2015 to Nov. 2016,216 patients with suspected chronic pancreatitis admitted to the Department of Gastroenterology of Beijing Changping DistrictHospital of Traditional Chinese Medicine were included in the study, all of which received EUS and CT examination. With fine needle biopsy results as gold standard, the diagnostic value of EUS and CT was compared. Results Of all 216 patients,136 cases were diagnosed as chronic pancreatitis,80 cases were non-pancreatitis. With needle biopsy as the gold standard,EUS sensitivity was 98.53%, specificity was 96.25%, accuracy was 98.53% ;CT sensitivity was 93.38%, specificity was 86.25%, accuracy was 93.38%. EUS examination： 24 cases of pancreatic duct dilated CP;42 cases of tumor type CP;40 cases of tumor ＋ bile duct dilated CP;28 cases of diffusive swell large CP; CT examination ：41 cases of tumor type CP ;39 cases of tumor ＋ bile duct dilated CP; 26 cases of diffuse swelling of CP; 21 cases of pancreatic duct dilated CP,pancreatic duct dilatation,pancreatic calculi/calcification. Conclusion The sensitivity and specificity of EUS for chronic pancreatitis are high, which can clearly reflect smaU changes in the parenchyma and pancreatic duct of the pancreas.
