超声相关生物力学技术在评价糖尿病患者左心室收缩功能中的应用 被引量:2

Application of Ultrasound Related Biomechanical Techniques in Assessing Left Ventricular Systolic Function in Patients with Diabetes Mellitus

摘要 糖尿病患者左心室收缩功能异常,预示患者发生心血管不良事件的危险较大。但常规超声检测指标,如左心室射血分数、左心室短轴缩短率等早期却无法准确反映左心室收缩功能的异常。近年来,随着生物力学及超声诊断技术的发展,应用无创生物力学检测指标,如运动速度、应变、应变率等来评价早期左心室收缩功能异常取得了很好的进展,但它们因受心脏几何结构、超声声束及帧频等影响,仍不能简单、全面及确切地应用于临床诊断糖尿病心肌病。而目前发展起来的心脏力学指标心肌应力,因可早期发现左心室功能及结构的异常,且所受影响因素较少,未来可用于糖尿病心脏病的早期诊断。 Abnormal left ventricular systolic function predicts a greater risk of cardiovascular adverse events in patients with diabetes mellitus. However, conventional ultrasonic testing indicators such as left ventricular ejection fraction and left ventricular short axis shortening rate can not accurately reflect the abnormal left ventricular systolic function in the early stage. In recent years, with the development of biomechanics and ultrasonic diagnosis technology, the application of noninva- sire biomechanical indexes, such as velocity, strain and strain rate to evaluate the left ventricular systolic function abnormality in early stage has made great progress, but due to the effects of heart geometric structure, ultrasonic beam and frame rate, it cannot realize simple, comprehensive and exact clinical application in the diagnosis of diabetic cardiomyopa- thy. The current developed mechanical indexes of myocardial stress, which can realize early detection of left ventricular dys- function and structural abnormalities, with less influencing factors, can be applied for early diagnosis of diabetic heart dis- ease in the future.
