摘要
目的观察血必净注射液联合无创机械通气对老年慢性阻塞性肺疾病(COPD)合并重症感染患者肺功能及血清炎症因子的影响。方法将92例老年COPD合并重症感染患者随机分为2组,对照组46例给予无创机械通气治疗,研究组46例给予血必净注射液联合无创机械通气治疗,2组均持续治疗7 d。比较2组治疗后临床疗效,检测2组治疗前后血气指标、肺功能指标以及炎症因子水平变化情况。结果治疗后,研究组治疗总有效率显著高于对照组(P<0.05);2组p(O2)均较治疗前显著升高(P均<0.05),p(CO_2)均较治疗前显著降低(P均<0.05),研究组p(O_2)均显著高于对照组(P均<0.05),p(CO_2)显著低于对照组(P<0.05);2组第1秒用力呼气量占预计值百分比(FEV1)、最大呼气流量(PEF)和肺活量(FVC)均较治疗前显著升高(P均<0.05),研究组均显著高于对照组(P均<0.05);2组C反应蛋白(CRP)、肿瘤坏死因子(TNF-α)和白细胞介素-6(IL-6)水平均较治疗前显著降低(P均<0.05),研究组均显著低于对照组(P均<0.05)。结论血必净注射液联合无创机械通气治疗老年COPD合并重症感染,可明显改善肺功能及血清炎症因子,疗效显著。
Objective It is to observe the effect of Xuebijing injection combined with non-invasive mechanical ventilation on pulmonary function and serum inflammatory factors in elderly patients with chronic obstructive pulmonary disease （COPD） combined with severe infection. Methods 92 cases of senile patients with COPD and severe infection were randomly divided into 2 groups, control group （46 cases） treated with non -invasive mechanical ventilation, and study group （46 cases） treated with Xuebijing injection ＋ non-invasive mechanical ventilation, both groups were continuously treated for 7 d. The clinical efficacy of the two groups was compared, and the changes of blood gas indexes, lung function indexes and levels of inflammatory factors were detected before and after treatment. Results After treatment, the total effective rate in the study group was significantly higher than that in the control group （P 〈 0.05 ） ; p（ O2 ） , FEV1, PEF, FVC were significantly increased （P 〈 0.05 ） , while p（ CO2 ） , the levels of CRP, TNF-α and IL-6 were decreased significantly in both groups after treatment （P 〈 0.05 ） , and the increase or decrease of these indexes in study group were more significant than those in control group （ P 〈 0.05 ）. Conclusion Xuebijing injection combined with non-invasive mechanical ventilation which can significantly improve pulmonary function and serum inflammatory factors has good curative effect on elderly COPD with severe infection.
