The Assessment of Immature Microvascular Density in Gliomas with Dynamic Contrast-enhanced Magnetic Resonance Imaging

摘要 目的:通过对脑胶质瘤患者进行动态对比增强磁共振成像(DCE-MRI)获得容积转运参数(Ktrans)与血管外细胞外间隙容积比(Ve),探讨DCE-MRI定量评估胶质瘤不成熟微血管密度的价值。方法:研究对象包括66例脑胶质瘤患者,其中Ⅱ级27例,Ⅲ级8例,Ⅳ级31例。每名脑胶质瘤患者术前经过DCE-MRI检查获得胶质瘤的Ktrans与Ve的最大值,术后胶质瘤标本进行免疫组织化学测量CD105阳性标记的微血管密度(CD105-MVD)的最大值。应用Mann-Whitney U检验比较不同级别胶质瘤Ktrans值、Ve值、CD105-MVD的差异,应用Pearman相关系数分析胶质瘤Ktrans值、Ve值与CD105-MVD的相关性。结果:低级别胶质瘤的Ktrans值、Ve值、CD105-MVD均小于高级别胶质瘤(P<0.001,P<0.001,P<0.001)。高级别胶质瘤的Ktrans值、Ve值均与CD105-MVD正相关(P<0.001,P<0.001)。结论:DCE-MRI可以定量评估脑胶质瘤不成熟微血管密度。 Purpose:To quantitatively assess the immature microvascular density (MVD) of brain gliomas with the volume transfer constant (Ktrans) and volume of extravascular extracellular space per unit volume of tissue (Ve) by dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI). Methods: Sixtysix patients with brain gliomas (grade Ⅱ, 27; grade Ⅲ, 8; and grade Ⅳ, 31)were included in this study. The maximal values of Ktrans and Ve from each patient were obtained before the operation. The maximal CD105- microvascular density (CD105-MVD) of each tumor was measured in the specimens. The differences of Ktrans, Ve and CD105-MVD between the different grades of gliomas were analyzed by the Mann-Whitney U-test. The Pearman correlation coefficient was determined between Ktrans, Ve and CD105-MVD. Results: The Ktrans, Ve and CD105-MVD of low-grade gliomas were significantly lower than those of high-grade gliomas (P〈0.001, P〈0.001, P〈0.001). Ktrans and Ve were positively correlated with CD105-MVD in high-grade gliomas (P〈0.001, P〈0.001). Conclusion: DCE-MRI can be used to assess the immature MVD of gliomas.
