摘要
目的观察无创通气对已综合评估为C组或D组AECOPD(慢性阻塞性肺病急性加重)合并Ⅱ型呼吸衰竭及伴焦虑、抑郁患者的临床疗效。方法 80例已综合评估为C组或D组的AECOPD合并Ⅱ型呼吸衰竭,并伴有焦虑及抑郁的患者,分别随机分为对照组、研究组,所有患者均给予常规的药物治疗;研究组在常规药物治疗基础上加用无创通气。结果研究组与对照组相比,Pa O2、FEV1%预计值升高更明显,Pa CO2下降更显著,PH值、呼吸频率更接近于正常值,同时焦虑评分、抑郁评分也更接近于正常值,上述几种影响因素与对照组相比均具有统计学意义(P<0.05)。结论无创通气对已综合评估为C组或D组AECOPD合并Ⅱ型呼吸衰竭及伴焦虑、抑郁的患者有明显的治疗效果。焦虑、抑郁也是评估AECOPD疗效的重要因素。
Objective To investigate the clinical efficacy of noninvasive ventilator on patients with general assessment C or D AECOPD complicated with type II respiratory failure, anxious and depression. Methods 80 pa-tients with general assessment C or D AECOPD complicated with type II respiratory failure, anxious and depression were randomly divided into two groups. All patients were given the conventional medical treatment, and the research group was additionally treated with noninvasive ventilator. Results After the treatment, the value of Pa02 and FEV! % estimated value were higher,the value of PaC02 was lower in the research group than in the control group. The value of PH and the respiratory rate were much closer to normal value, and the anxiety score and the depression score were much closer to normal value too. The influencing factors of the above were statistically significant （P 〈 0. 05 ） . Conclusion Noninvasive ventilator has a significant curative effect on patients with general assessment C or D AECOPD complicated with type II respiratory failure, anxious and depression. Anxiety and depression are also im-portant factors in assessing the clinical efficacy of AECOPD.
