摘要
目的对比分析依那普利联合硝苯地平控释片对老年冠心病合并高血压患者的临床疗效。方法选取第四军医大学西京医院2014年3月至2016年3月收治入院的60岁以上老年冠心病合并高血压患者80例作为研究对象,随机分为研究组和对照组,每组各40例。对照组患者给予硝苯地平控释片治疗,研究组患者采用依那普利联合硝苯地平控释片治疗。比较两组患者治疗前后血肌酐、血钾、血尿酸水平;观察两组患者治疗前后舒张压、收缩压、脉压差变化及治疗过程中缺血状态发生率,并分析药物不良反应和临床疗效。结果研究组患者治疗后血肌酐和血钾水平均显著高于本组治疗前和对照组治疗后(P<0.05),尿酸水平显著低于本组治疗前和对照组治疗后(P<0.05)。研究组患者治疗总有效率显著高于对照组(χ~2=5.5411,P=0.0186),治疗后收缩压、舒张压及脉压差均显著低于对照组(P<0.05)。两组患者治疗后的收缩压、舒张压及脉压差均低于本组治疗前(P<0.05)。研究组患者缺血状态总发生率显著低于对照组(χ~2=7.3837,P=0.0066)。两组患者不良反应发生率无显著差异(P>0.05)。结论依那普利联合硝苯地平控释片可以降低老年冠心病合并高血压患者的血压,提高临床疗效,且安全性较高,值得临床参考。
Objective To compare the efficacy of Enalapril combined with Nifedipine controlled release tablets in the treatment of elderly patients with coronary heart disease complicated with hypertension.Method Selected 80 patients treated in Xijing Hospital from March 2014 to March 2016 as objects of study, all patients were randomly divided into study group （40 cases） and control group （40 cases）. Control group patients were treated with Nifedipine controlled release tablets only, study group patients were treated with Enalapril combined with Nifedipine controlled release tablets. Serum creatinine, serum potassium and serum uric acid levels were compared before and after treatment between the two groups. The changes of systolic pressure （SBP）, diastolic pressure （DBP）, pulse pressure and ischemic status during treatment, adverse drug reactions and clinical curative effects were analyzed.Result After treatment, the levels of serum creatinine and serum potassium in study group were significantly higher than those before treatment and control group （P〈0.05）, the levels of serum uric acid was significantly lower than before treatment and control group （P〈0.05）. The total effective rate in study group was significantly higher than that in control group （χ^2=5.5411,P=0.0186）. The SBP, DBP, pulse pressure in study group were significantly lower than control group （P〈0.05）. After treatment, SBP, DBP, pulse pressure reduced significantly in both groups （P〈0.05）. The ischemic status in study group was significantly lower than that in control group （χ^2=7.3837,P=0.0066）. There was no significantly difference in adverse drug reactions between the two groups （P〉0.05）.Conclusion The combination of Nifedipine controlled release tablets and Enalapril in the treament of elderly patients with coronary heart disease complicated with hypertension, can not only improve the symptoms of hypertension, reduce the blood pressure level, but also improve the clinical curative effect, and the safety is high, it is worthy of clinical reference.
