摘要
目的探讨经颅多普勒超声(TCD)对外伤性脑梗死(PTCI)发生发展及转归的检测作用,为PTCI的预防、早期诊断及治疗提供参考。方法对254例重型颅脑伤患者行动态TCD及头颅CT检查,必要时行头颅MR及CTA检查,对数据进行统计分析。结果 CT及MR证实共发生脑梗死29例,且早于CT明确脑梗死前,TCD即提示脑供血不足,TCD参数变化有统计学意义(P<0.01)。治疗过程中,TCD检测脑血流及时改善者,脑梗死多有不同程度恢复,血流状况无好转者,脑梗死进一步加重。结论 TCD能检测到PTCI发生之前的血流异常及治疗过程中血流的恢复情况,对PTCI的预防、早期诊断及治疗有十分重要的指导意义。
Objective To investigate the utility of TCD monitoring on prophylaxis,early diagnosis and treatment of post-traumatic cerebral infarction( PTCI). Methods 254 patients with fulfilled clinical criteria of severe craniocerebral trauma underwent careful examination by dynamic TCD and cranial computed tomography,cranial MR and CTA were also performed when necessary.Data was compiled and analyzed by statistical method. Results 29 cases were confirmed as cerebral infarction by cranial computed tomography in this study,however,TCD revealed insufficient of cerebral blood supply with significant difference in the diagnostic parameter( P〈 0. 01) prior to the confirmation of cerebral infarction by CT or MR. During the treatment course,the cerebral infarction showed recovered at various degrees as the cerebral blood supply improved in time monitored by TCD,while it may become worse when blood supply did not show improved.Conclusion Through monitoring the abnormity of blood flow before PTCI and the improvement during treatment course,TCD may play an important role in the prophylaxis,early diagnosis and treatment on PTCI.
