振幅整合脑电图联合婴儿智能发育量表在早产儿脑损伤中的诊断价值 被引量:18

Value of aEEG combined with CDCC in diagnosing preterm infants with brain injury

摘要 目的评估振幅整合脑电图(aEEG)联合婴儿智能发育量表(CDCC)在早产儿脑损伤中的诊断价值。方法存在脑损伤高危因素的早产儿78例分为脑损伤组(48例)及非脑损伤组(30例),于生后6h内行aEEG描记,生后7~10d行头颅MRI检查,纠正胎龄3个月行CDCC检查,分析aEEG、CDCC及其联合运用在早产儿脑损伤中的诊断价值。结果 aEEG、CDCC及其联合运用预测早产儿脑损伤的灵敏度分别为87.50%、66.67%、92.09%,特异度分别为76.67%、86.66%、66.44%,ROC曲线下面积分别为0.821、0.767、0.840。与aEEG、CDCC单独检测比较,aEEG联合CDCC诊断早产儿脑损伤的灵敏度最高,ROC曲线下面积最大(P<0.05)。结论 aEEG联合CDCC检查可作为早期诊断早产儿脑损伤的有效手段。 Objective To evaluate the value of amplitude integrated electroencephalogram(aEEG)combined with children’s developmental center of China(CDCC)in the diagnosis of preterm infants with brain injury.Methods Eighty-five preterm infants with risk factors of brain injury were divided into two groups of A(with brain injury,48 cases)and B(without brain injury,30 cases).The aEEG was traced within 6 hours after birth,cerebral MRI was performed during 7 to 10 days of age,and CDCC examination was performed in the corrected age of 3 months.The diagnostic values of aEEG alone,CDCC alone and combined use of both were analyzed.Results In the diagnosis of brain injury,the areas under the ROC curve were 0.821,0.767 and 0.840 for aEEG alone,CDCC alone and combined use of both(P〈0.05)with the sensitivities of 87.50%,66.67% and 92.9% and the specificities of 76.67%,86.66% and 66.44%,respectively.Conclusion Combined use of aEEG and CDCC can be taked as a useful tool for early diagnosis of brain injury in preterm infants.
