摘要
目的探讨卡泊三醇联合窄谱紫外线治疗银屑病的效果和机制。方法方便选择2015年2月—2016年7月在该院接受治疗的86例银屑病患者随机分为对照组与观察组,每组43例。对照组给予窄谱中波紫外线(NB-UVB)治疗,观察组给予卡泊三醇联合NB-UVB治疗。比较两组患者治疗效果和T细亚群水平变化情况。结果观察组治疗总有效率为95.3%,明显高于对照组的76.7%(P<0.05)。对照组患者治疗后CD4+、CD8+无明显变化,CD4+/CD8+明显升高;观察组治疗后CD4+为(40.2±6.5)%,明显高于治疗前的(32.9±8.4)%,CD8+为(21.4±7.1)%明显低于治疗前(29.1±6.5)%,CD4+/CD8+为(1.94±0.18),明显高于治疗前的(1.06±0.51),且三项指标与对照组治疗后比较差异有统计学意义(P<0.05)。结论 NB-UVB联合卡泊三醇能够提高银屑病的治疗效果,有效促进患者T淋巴细胞免疫功能的恢复。
Objective To discuss the effect and mechanism of calcipotriol and narrow band ultraviolet on the T cell subgroup level of patients with psoriasis. Methods Convenient selection 86 cases of patients from Feburary 2015 to July 2016 with psoriasis treated in our hospital were randomly divided into two groups with 43 cases in each, the control group were treated with NB-UVB, while the observation group were treated with calcipotriol and narrow band ultraviolet, and the treatment effect and changes of T cell subgroup levels were compared between the two groups. Results The total treatment effective rate in the observation group was obviously higher than that in the control group（95.3% vs 76.7%）,（P〈0.05）, and there were no obvious changes in the CD4＋ and CD8＋ after treatment in the control group, but CD4＋/CD8＋ obviously increased,after treatment, the CD4＋ in the observation group was obviously higher than that before treatment, [（40.2±6.5）% vs（32.9±8.4）%], and CD8＋ was obviously lower than that before operation [（21.4±7.1）% vs（29.1±6.5）%], and CD4＋/CD8＋ was（1.94±0.18）, which was obviously higher than that before operation（1.06±0.51）, and the differences in the three indexes between the two groups after treatment were obvious（P〈0.05）. Conclusion NB-UVB and calcipotriol can improve the treatment effect of psoriasis, which can effectively promote the recovery of T lymphocyte immune function of patients.
