摘要
目的探讨对穿孔性阑尾炎患者行腹腔镜阑尾切除术和开腹阑尾切除术治疗的临床效果。方法方便选取2015年8月—2016年8月该院收治的84例穿孔性阑尾炎患者作为研究对象,根据患者住院病床号码进行分组研究,单号为开腹组,双号为腹腔镜组,各42例。开腹组行常规的开腹阑尾切除术治疗,腹腔镜组行腹腔镜阑尾切除术治疗,观察并比较两组发生并发症几率、各项手术指标。结果腹腔镜组术中出血量(25.3±8.20)m L、手术时间(63.3±11.50)min、切口长度(2.3±0.10)cm、下床时间(1.7±0.50)d、首次肛门排气时间(1.8±0.60)d、首次进食时间(1.62±0.33)d、住院时间(2.11±1.03)d均低于开腹组(49.8±10.40)m L、(105.3±10.50)min、(7.4±2.70)cm、(2.5±1.40)d、(2.9±1.40)d、(2.59±0.48)d、(10.15±2.15)d。腹腔镜组发生术后并发症几率3例(7.14%)显著低于开腹组12例(28.57%),差异有统计学意义(P<0.05)。结论对穿孔性阑尾炎患者行腹腔镜手术,能够在一定程度上加快患者的恢复速度,同时,还能够降低术后并发症的发生几率。
Objective To study the clinical effect of Laparoscopic and conventional appendectomy in treatment of perforating appendicitis. Methods 84 cases of patients with perforating appendicitis treated in our hospital from August 2015 to August 2016 were selected and randomly divided into two groups with 42 cases in each, the laparoscopic group and conventional appendectomy group respectively adopted the routine laparoscopic surgery and appendectomy surgery, and the complication and various operation indexes were compared between the two groups. Results The intraoperative bleeding amount,operation time, incision length, time to get out bed, first anal exhaust time, first dieting time and length of stay were lower than those in the appendectomy group [(25.3±8.20)m L,(63.3±11.50)min,(2.3±0.10)cm,(1.7±0.50)d,(1.8±0.60)d,(1.62±0.33)d,(2.11±1.03)d vs(49.8±10.40)m L,(105.3±10.50)min,(7.4±2.70)cm,(2.5±1.40)d,(2.9±1.40)d,(2.59±0.48)d,(10.15±2.15)d], and the incidence rate of complications after operation was obviously lower than that in the appendectomy group(7.14% vs 28.57%, 3 cases vs 12 cases), and the differences statistically significant(P〈0.05). Conclusion Laparoscopic surgery for perforating appendicitis patients can speed up the recovery speed to a certain degree and reduce the occurrence probability of postoperative complications.
