摘要
目的:探讨血清胱抑素C(CysC)、25-羟维生素D[25(OH)D]与2型糖尿病(T2DM)患者颈动脉内膜中层厚度(CIMT)的相关性。方法:选择2015年2月-2017年2月笔者所在医院内分泌科住院的292例T2DM患者,根据颈部血管彩超测得CIMT结果分为CIMT正常组(n=87)、CIMT增厚组(n=150)及斑块组(n=55),并对CIMT增厚的相关危险因素进行多因素Logistic回归分析,利用ROC曲线分析相关指标对CIMT增厚的预测价值。结果:糖尿病病程、CysC是CIMT增厚的独立危险因素,而25(OH)D是其保护因素(P<0.05);ROC曲线示,CysC曲线下面积为0.848,最佳阈值为1.46 mg/L,敏感度为0.856,特异度为0.798;25(OH)D曲线下面积为0.836,最佳阈值为26.38 ng/ml,敏感度为0.844,特异度为0.752。结论:血清CysC与25(OH)D对CIMT增厚均具有较高的预测价值。
Objective:To observe the relationship between cystatin C(CysC),25-hydroxyvitamin D[25(OH)D] and carotid intima-media thickness(CIMT) in type 2 diabetes mellitus(T2DM) patients.Method:292 cases with T2 DM were selected to the endocrinogical department of our hospital from February 2015 to February 2017,all subjects received neck vascular ultrasound examination.Patients were divided into three groups based on the CIMT value:CIMT normal group(n=87),CIMT thickening group(n=150) and plaque group(n=55).A multi-factor Logistic regression was performed to analyze the risk factors for the increase of CIMT,and the predictive value was verified on the risk factors by receiver operating characteristic(ROC) curve.Result:Disease course,CysC were the independent risk factors for the increasing of CIMT,while 25(OH)D was the protective factor(P〈0.05).ROC curve analysis revealed that the optimal cutoff of CysC to predict the increasing of CIMT was 1.46 mg/L(AUC=0.848,sensitivity 0.856,specificity 0.798),and the optimal cutoff of 25(OH)D to predict that was 26.38 ng/ml(AUC=0.836,sensitivity 0.844,specificity 0.752).Conclusion:It is proved that CysC and 25(OH)D have a higher predictive value in the increasing of CIMT.
