摘要
目的探讨长链非编码RNA人类白细胞抗原复合体18（HCG18）在人胃癌组织表达及临床意义。方法收集人胃癌和配对正常胃黏膜组织，采用荧光实时定量聚合酶链反应（PCR）方法检测组织中HCG18表达水平，分析其表达与患者性别、年龄、肿瘤部位、肿瘤直径、组织分化程度、肿瘤浸润深度、淋巴结转移、远处转移、TNM分期及脉管内有无癌栓等临床病理因素的关系。结果在108例胃癌组织中HCG18阳性率为77．78％（90／108），而正常胃组织HCG18阳性率为2．78％（3／108）。x^2=142．9，P=0．000，差异有统计学意义。进一步分析HCG18与患者性别、年龄、肿瘤部位、肿瘤直径、组织分化程度、肿瘤浸润深度、淋巴结转移、远处转移、TNM分期及脉管内有无癌栓等临床病理因素的关系。HCG18表达量与患者性别（r=1．122，P=0．289）、年龄（x^=2．233，P=0．135）、肿瘤位置（x^=0．302，P=0．960）、组织分化程度（x^2=1．218，P=0．544）均无明显相关，但与肿瘤直径（x^2=10．072，P=0．002）、肿瘤浸润深度（x^2=9．874，P=0．002）、淋巴结转移（x^2=5．214，P=0．022）、远处转移（x^2=4．800，P=0．028）、TNM分期（x^2=12．115，P=0．007）以及脉管内有无癌栓（x^2=7．902，P=0．005）均显著相关。结论检测HCG18有助于胃癌基因诊断。
Objective To investigate the clinical significance of long chain noncoding human leu- cocyte antigen complex group 18 （HCG18） RNA in human gastric carcinoma. Methods Human gastric cancer and matched normal gastric tissues were collected, and HCG18 was detected by fluorescence quanti- tative realtime polymerase chain reaction （PCR）. The relationship between HCG18 expression and patient＇ s sex, age, tumor location, tumor diameter, histological differentiation, depth of tumor invasion, lymph node metastasis, distant metastasis, TNM staging and vascular tumor thrombus and other clinico- pathological factors was analyzed, respectively. Results The results showed that HCG18 expression was not significantly correlated with the patient gender （x^2 = 1. 122, P = 0. 289 ）, age （x^2 = 2. 233, P = 0. 135 ）, tumor location （x^2= 0. 302, P = 0. 960 ）, histological differentiation （x^2 = 1. 218, P = 0. 544）, but significantly correlated with tumor diameter （x^2 = 10. 072, P = 0. 002）, tumor invasion depth （x^2 = 9. 874 ,P =0.002）, lymph node metastasis （x^2 = 5. 214 ,P =0.022）, distant metastasis （x^2=4. 800 ,P = 0. 028）, TNM staging （x^2 = 12. 115, P = 0. 007 ） and tumor thrombus （x^2 = 7. 902, P = 0. 005 ）. HCG18 expression level was one of the important independent risk factors influencing the prognosis of patients with gastric cancer. Conclusion Detection of HCG18 is helpful for the gene level diagnosis of gastric cancer.
