摘要
目的观察阿奇霉素鼻腔灌洗对慢性鼻窦炎鼻内镜术后术腔黏膜修复过程的影响。方法60例慢性鼻窦炎患者随机分为三组,每组20例,分别为阿奇霉素混合液组(阿奇霉素+布地奈德混悬液+氨溴索)、生理盐水组、对照组。前两组分别进行相应的鼻腔灌洗,疗程1月,并随访6月。观察比较鼻内镜术后术腔黏膜修复过程及相关临床症状、体征改善程度,评价其疗效。结果术后3个月时,混合液组、生理盐水组、对照组患者总有效率分别为90%、75%、70%;术后6个月时三组患者总有效率分别为90%、85%、80%;组间比较差异有统计学意义(P<0.05)。鼻腔黏膜上皮化平均时间为混合液组48.4±6.5d,生理盐水组58.3±7.5d,对照组61.3±7.5d,组间比较差异有统计学意义(P<0.01)。结论阿奇霉素混合液鼻腔灌洗在慢性鼻窦炎患者内镜术后术腔黏膜恢复过程中具有明显促进作用,值得在临床推广应用。
Objective To observe the effect of nasal irrigation with compound azithromycin solution on the recovering process of mucosa in sinus cavity with chronic lesion treated by endoscopic surgery based on a clinical trial. Methods Sixty cases with typeⅡchronic sinusitis undergone FESS were included in this study, randomly divided into three groups, i.e. treating group （TG） treated by nasal irrigating with compound azithromycin solution, controlling group （CG） treated by nasal irrigating, and blank group （BG） given no nasal irrigating, with 20 cases in each. Then, their therapeutic effects were evaluated in comparative way by the end of following up period last for 6 months. Results The total effective rates were 85%, 75% and 70% for TG, CG and BG by the end of 3rd month during the following up period, and 90%, 85%and 80%for them by the end of 6th month during the following up period respectively, with statistical significance among them （P〈0.05）. Furthermore, the mean time of mucosal epithelization in operated cavity was 48.4± 6.5 days for cases in TG, 58.3 ±7.5 days for ones in CG, and 61.3 ±7.5 days for those in BG, with markedly statistical significance among them as well （P〈0.01）. Conclusions Compound azithromycin solution holds significantly improving effect on the recovery process of mucosa in operated sinus cavity of cases with chronic sinusitis following nasal endoscopic surgery when used in a way of nasal irrigation. Therefore, it is worthy to be promoted into practical usage in clinic.
