摘要
目的:观察和评价益脾养肝方联合TACE和3DCRT治疗原发性肝癌的临床疗效。方法:51例原发性肝癌患者随机分为治疗组(26例),给予中药益脾养肝方联合TACE及3DCRT治疗;对照组(25例),给予TACE和3DCRT治疗。两组疗程35~42 d(根据3DCRT治疗时间而变化)。观察和比较两组病例在治疗前后中医证候、实体瘤大小、细胞免疫功能、肝功能、生存率等方面的相关指标并进行统计分析。结果:经治疗后,治疗组中医疗效总有效率88.0%,对照组中医疗效总有效率32.0%,两组比较有显著性差异(P<0.05)。治疗组在中医证候改善,CD3+、CD3+CD4+变化,降低ALT、AST等方面优于对照组,差异有统计学意义(P<0.05)。治疗组在实体瘤大小改变、生存率等方面均优于对照组,但差异无统计学意义(P>0.05﹚。结论:益脾养肝方联合TACE和3DCRT治疗原发性肝癌,可以明显改善患者临床症状、改善生化指标、提高患者生活质量,且安全可靠。
Objective ： The clinical efficacy in the treatment of primary hepatocellular carcinoma with the Decoction of Strength- ening Spleen and supplementing Liver combined with TACE and 3 DCRT was evaluated. Methods：Fifty - one patients with primary hepatocellular carcinoma were randomly divided into 2 groups：control group （25 cases） and treatment group （26 cases）. All pa- tients were treated with TACE and 3DCRT. The Decoction of Strengthening Spleen and Supplementing Liver was to be added to the treatment group. And then detect the clinical symptoms changes, tumor size, cellular immune function, liver function, survival rate and other aspects of related indicators and statistical analysis. Results：After the course of treatment,88 % of patients in treatment group were cured compared with a percentage of 32% in control group and there was a significant statistical difference between the two groups（ P 〈 0. 05 ）. And in clinical symptoms, the immunity cells （ CD3 ^＋ , CD3 ^＋ CD4 ^＋ ） and liver function （ ALT, AST） were significantly improved with a signigcant difference between the two groups （ P 〈 0. 05 ）. In tumor size, survival rates were significantly improved and there was no significant statistical difference between the two groups （P 〉 0. 05 ）. Conclusion：The method which was treating primary hepatocellular carcinoma by the Decoction of Strengthening Spleen and Supplementing Liver combined with TACE and 3DCRT can significantly improve the clinical symptoms, the biochemical indicators and the quality of life of patients and had a better curative effect.
