摘要
目的分析PFNA内固定治疗不稳定型股骨转子间骨折的疗效。方法选择我院2015年1月~2016年1月住院治疗的不稳定型股骨转子间骨折患者共60例,其中30例行PFNA内固定治疗(PFNA组),另30例行动力髋螺钉(DHS)治疗作为对照组,对两组患者的平均手术时间、术中出血量、切口长度、骨折愈合时间进行比较,并对两组患者的髋关节功能恢复的优良率进行评价。结果 PFNA组患者的术中出血量、切口长度、平均手术时间、骨折愈合时间与对照组比较,差异有统计学意义(P<0.05),结果显示,PFNA组患者的术中出血少、切口小、手术时间短、患者术后骨折愈合快。PFNA组患者的髋关节功能优良率为70%(21/30),对照组的髋关节功能优良率为50%(15/30),组间比较差异有统计学意义(P<0.05)。结论 PFNA内固定治疗不稳定型股骨转子间骨折可以取得较好的治疗效果,与DHS内固定比较具有手术切口小、出血少、并发症少、骨折愈合快、髋关节功能恢复快等优势,值得推广应用。
Objective To evaluate the efficacy of PFNA internal fixation in the treatment of unstable intertrochanteric fractures. Methods A total of 60 patients with unstable intertrochanteric fractures were selected from January 2015 to January 2016, 30 cases underwent PFNA internal fixation （PFNA group）, another 30 patients treated with dynamic hip screw（DHS） were selected as the control group, the average operation time, intraoperative blood loss, incision length, fracture healing time and the excellent and good rate of hip function were compared in two groups. Results The differences of intraoperative blood loss, length of incision, the average operation time, healing time of fracture in PFNA group were statistically significant（P〈O.05）. It showed that the PFNA group of patients with less intraoperative bleeding, smaller incision, shorter operation time, faster fracture healing. The excellent and good rate of hip function of patients in PFNA group was 70%（21/30）, the control group was 50%（15/30）, and there was significant difference（P〈0.05）. Conclusion PFNA internal fixation in treatment of unstable intertrochanteric fractures has good effect, with smaller incision, less bleeding, fewer complications, quicker fracture healing, hip joint function recovery and other advantages compared to DHS internal fixation, is worthy of promotion and application
