摘要
目的探讨低温等离子髓核消融术治疗颈性眩晕的临床疗效。方法选择我院骨科于2014年1月至2015年8月期间收治的32例颈性眩晕患者为观察组,给予低温等离子髓核消融术治疗,随机选择同期保守治疗的32例患者纳入对照组,比较两组患者治疗前后眩晕及伴随症状的数字评分法(NRS)评分及临床疗效。结果观察组患者手术过程顺利,未出现严重并发症;观察组患者治疗后眩晕及头痛、恶心呕吐、耳鸣、视物障碍、颈肩痛、心悸胸闷等NRS评分分别为(1.36±0.59)分、(2.24±0.98)分、(1.52±0.67)分、(1.05±0.52)分、(1.38±0.63)分、(1.13±0.54)分、(1.22±0.48)分,对照组分别为(3.46±1.74)分、(4.32±1.48)分、(3.32±1.58)分、(2.25±1.02)分、(2.38±0.96)分、(4.23±1.74)分、(2.42±0.98)分,两组患者上述评分均较治疗前明显下降,且观察组低于对照组,差异均有统计学意义(P<0.05);观察组患者治疗后的优良率为90.63%,有效率为96.88%,随访1年时,优良率为87.50%、有效率93.75%,均显著高于对照组的50.00%、75.00%和40.63%、68.75%,差异均有统计学意义(P<0.05)。结论低温等离子髓核消融术治疗颈性眩晕可以快速改善患者的眩晕及伴随症状,疗效确切,且具有较好的安全性。
Objective To study the clinical effect of low-temperature plasma ablation of nucleus pulposus on cervical vertigo. Methods Thirty-two patients with cervical vertigo treated in Department of Orthopedics from January2014 to August 2015 were treated with low-temperature plasma ablation of nucleus pulposus（the observation group）,and 32 patients with conservative treatment were randomized into the control group. The Numerical Rating Scale（NRS）scores of vertigo and concomitant symptoms before and after treatment were compared, and the clinical efficacy was recorded. Results The operation course of the observation group was smooth and no serious complications occurred. The NRS scores of headache, nausea and vomiting, tinnitus, visual impairment, neck and shoulder pain, palpitations and chest tightness in the observation group were（1.36±0.59）,（2.24±0.98）,（1.52±0.67）,（1.05±0.52）,（1.38±0.63）,（1.13±0.54）,（1.22 ± 0.48）, as compared with（3.46 ± 1.74）,（4.32 ± 1.48）,（3.32 ± 1.58）,（2.25 ± 1.02）,（2.38 ± 0.96）,（4.23 ± 1.74）,（2.42±0.98） in the control group（P〈0.05）, which were all significantly lower than those before treatment（P〈0.05）. After treatment, the excellent and good rate, the effective rate of the observation group was 90.63%, 96.88%, as compared with 50.00%, 75.00% in the control group（P〈0.05）. After one year of follow-up, the excellent and good rate, the effective rate was 87.50%, 93.75% in the observation group, as compared with 40.63%, 68.75% in the control group（P〈0.05）. Conclusion Low-temperature plasma ablation of nucleus pulposus for the treatment of cervical vertigo can rapidly improve the patients＇ vertigo and concomitant symptoms. The curative effect is definite and has good safety.
