摘要
[目的]观察调脂方联合小剂量辛伐他汀治疗维持性血液透析脂代谢紊乱疗效。[方法]使用随机平行对照方法,将70例门诊及住院患者按随机数字表法随机分为两组,常规维持血液透析。对照组35例辛伐他汀片20mg/d,晚间顿服。治疗组35例辛伐他汀片10mg/d,晚间顿服;调脂方(葛根、玉竹、神曲各15g,当归12g,山楂15g,大黄炭10g,丹参15g,女贞子12g),1剂/d,水煎250ml,早晚温服。观测临床症状、血清胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白(HDL-C)、低密度脂蛋白(LDL-C)、不良反应。治疗1疗程(3个月),判定疗效。[结果]治疗组痊愈2例,显效12例,有效17例,无效4例,总有效率88.57%;对照组痊愈1例,显效9例,有效16例,无效9例,总有效率74.29%;治疗组疗效优于对照组(P<0.05)。TC、TG、HDL-C两组均有改善(P<0.05),治疗组改善优于对照组(P<0.05),LDL-C治疗组明显改善(P<0.05),对照组无明显变化(P>0.05)。[结论]调脂方联合小剂量辛伐他汀能有效降低血液透析患者血脂水平。
[Objective] To observe the therapeutic effect of lipid regulating prescription combined with low dose simvastatin on lipid metabolism disorder in maintenance hemodialysis patients. [Method] 70 cases of outpatients and inpatients were randomly divided into two groups according to random parallel control method. The control group of 35 cases of Simvastatin Tablets 20 mg/d, evening meal. The treatment group of 35 cases of Simvastatin Tablets 10 mg/d, the evening meal; Tiaozhi Decoction （Ge Gen, polygonatum, Divine Comedy 15g, angelica 12g, hawthorn 15g, rhubarb charcoal 10g, Salvia 15g, Ligustrum 12g）, 1 agent/d, decoction 250mL, sooner or later. Clinical symptoms, serum cholesterol （TC）, triglyceride （TG）, high density lipoprotein （HDL-C）, low density lipoprotein （LDL-C）, adverse reactions were observed. Treatment 1 course of treatment （for 3 months）, judge curative effect. [Results] The treatment group was cured in 2 cases, markedly effective in 12 cases, effective in 17 cases, ineffective in 4 cases, the total effective rate was 88.57%; the control group recovered 1 cases, markedly effective in 9 cases, effective in 16 cases, 9 cases, the total effective rate was 74.29%.TC, TG, HDL-C two groups were improved （P〈0.05）, the treatment group improved better than the control group（P〈0.05）, LDL-C treatment group was significantly improved （P〈0.05）, the control group had no significant change（P〉0.05）. [Conclusion] The prescription of regulating lipid combined with small dose of simvastatin can effectively reduce blood lipid level in hemodialysis patients.
