摘要
目的探讨老年高血压病人室间隔厚度与颈动脉内膜中层厚度(IMT)、血管内皮功能的相关性。方法选择2015年1月至2016年1月荆州市第一人民医院收治的老年高血压病人100例,根据室间隔厚度将病人分为肥厚组(≥12 mm)62例和非肥厚组(<12 mm)38例,同时选择50例健康志愿者作为对照组,检测各组室间隔厚度、IMT,采用肱动脉血流介导的舒张功能(FMD)和血清内皮素-1(ET-1)、一氧化氮(NO)水平评价血管内皮功能,分析室间隔厚度与IMT、血管内皮功能各指标的相关性。结果方差分析显示,3组IMT、室间隔厚度、FMD、ET-1和NO水平比较差异有统计学意义(P<0.05),其中肥厚组IMT、室间隔厚度、ET-1显著高于非肥厚组和对照组,FMD、NO水平显著低于非肥厚组和对照组,非肥厚组IMT、室间隔厚度、ET-1水平显著高于对照组,FMD、NO水平显著低于对照组,差异均有统计学意义(P<0.05)。Pearson相关分析显示,高血压病人室间隔厚度与IMT和ET-1呈正相关,而与FMD、NO呈负相关,差异有统计学意义(P<0.05)。结论部分老年高血压病人存在室间隔肥厚,且室间隔肥厚的老年高血压病人颈动脉和血管内皮功能损害更严重。
Objective To explore the correlation of ventricular septal thickness with carotid intima-media thickness( IMT) and vascular endothelial function in elderly patients with hypertension.Methods One hundred elderly hypertensive patients were chosen from January 2015 to January 2016 in First People's Hospital of Jingzhou City,and they were divided into hypertrophy group( ≥12 mm) and non-hypertrophy group( 〈12 mm) according to the ventricular septal. Fifty cases of healthy volunteers were selected as control group. The ventricular septal thickness,IMT,flow-mediated dilation( FMD),endothelin 1( ET-1) and nitric oxide( NO) were detected. The correlation between ventricular septal thickness and IMT or vascular endothelial function were analyzed. Results Variance analysis showed that the difference in IMT,interventricular septal thickness,FMD,ET-1 and NO among three groups was statistically significant( P〈0. 05). The levels of IMT,interventricular septal thickness and ET-1 in hypertrophy group were significantly higher than those in non-hypertrophy group and control group,and the levels of FMD and NO were significantly lower than those in the non-hypertrophy group and control group. The levels of IMT,interventricular septum thickness and ET-1 in no hypertrophy group were significantly higher than those in the control group,and the levels of FMD and NO were significantly lower than those in control group( P〈0. 05). Pearson correlation analysis showed that interventricular septum thickness was significantly positively related to IMT and ET-1,while significantly negatively related to FMD,NO in patients with hypertension( P〈0. 05). Conclusions Elderly hypertensive patients with ventricular septum hypertrophy has more serious damage of carotid artery and vascular endothelial function than elderly hypertensive patients without ventricular septum hypertrophy.
