摘要
目的探究腺苷蛋氨酸联合多烯磷脂酰胆碱治疗妊娠期胆汁淤积症(ICP)患者的临床疗效。方法选取2015年4月至2016年10月收治的70例ICP患者,采用随机数字表法分为试验组和对照组,各35例。对照组单纯采用腺苷蛋氨酸治疗,试验组采用腺苷蛋氨酸联合多烯磷脂酰胆碱治疗,对比两组治疗前后的肝功能指标及妊娠结局情况。结果治疗前两组肝功能指标比较,差异无统计学意义(P>0.05);治疗后两组的门冬氨酸转氨酶(ALT)、丙氨酸转氨酶(AST)、总胆红素(TB)、直接胆红素(DB)、血清总胆汁酸(TBA)均显著降低,组间比较,差异有统计学意义(P<0.05);试验组不良妊娠结局率为31.4%(11/35),产后出血发生率为11.4%(4/35),对照组不良妊娠结局率为62.9%(22/35),产后出血发生率为25.7%(9/35),差异有统计学意义(P<0.05)。结论采用腺苷蛋氨酸联合多烯磷脂酰胆碱方案治疗ICP患者的疗效优于单独应用腺苷蛋氨酸,能够有效改善患者的肝功能,减少不良妊娠结局的发生率。
Objective To investigate the clinical efficacy of adenosylmethionine combined with polyenephosphatidylcholine in the treatment of cholestasis of pregnancy. Methods 70 patients with cholestasis of pregnancy were randomly divided into experimental group and control group, 35 cases of each group. The patients in the control group were treated with adenosylmethionine alone, In the experimental group, adenosylmethionine combined with polyenephosphatidylcholine was used to compare the liver function and the outcome of the two groups before and after treatment. Results There was no significant difference between the two groups in the liver function before treatment(P0.05). The levels of ALT, AST, TB, DB and TBA in the experimental groups were significantly lower than those in the control group after treatment(P0.05). In the experimental group, the incidence of adverse pregnancy was 31.4%(11/35) and the postpartum hemorrhage was 11.4%(4/35). In the control group, the incidence of adverse pregnancy was62.9%(22/35) and the postpartum hemorrhage was 25.7%(9/35). There were statistically significant difference between the two groups(P0.05). Conclusion Adenosylmethionine combined with polyenephosphatidylcholine is more effective than adenosylmethionine alone in the treatment of cholestasis of pregnancy. It can effectively improve the liver function and reduce the incidence of adverse pregnancy outcomes.
