摘要
目的探讨无保护会阴接生联合硬膜外镇痛在初产妇中的应用价值。方法选择2015年12月~2016年11月我院收治的经阴道分娩初产妇80例,采用随机数字表法将其分为对照组和观察组,各40例。对照组实施经阴道分娩,并行无保护会阴接生技术,观察组在对照组基础上应用持续硬膜外镇痛。比较两组产妇不同时间点疼痛程度、恢复正常性生活时间、性生活时性唤起时间及每月性生活次数,统计分娩过程中会阴裂伤情况。结果观察组产妇的第二产程、产后24 h及出院时的疼痛视觉模拟评分均低于对照组(P<0.01),会阴裂伤率及会阴切开率均低于对照组(P<0.05),恢复正常性生活时间早于对照组(P<0.01),性唤起时间快于对照组(P<0.01),性生活频率高于对照组(P<0.01)。结论无保护会阴接生联合持续硬膜外分娩镇痛技术能有效减少初产妇分娩疼痛,降低会阴裂伤及侧切率,提高术后性生活质量。
Objective To study the clinical value of unprotected perineum delivery combined with continuous epidural analgesia in primipara.Methods 80 primiparous women undergoing transvaginal delivery from December 2015 to November 2016 were enrolled in this study,and they were divided into control group and observation group by the random number table method,40 cases in each group.The control group was underwent vaginal delivery and unprotected perineum delivery,and the observation group was treated with continuous epidural analgesia on the basis of the treatment of the control group.Then the pain degree at different time point,the time of restoring sexual life,sexual arousal time and sex once a month were compared between the two groups,and the perineal laceration during the delivery of the two groups was counted.Results In the second stage of labor,24 h after delivery and at the time of discharge,the pain visual analogue scores in the observation group was significantly lower than that of the control group（P〈0.01）;the time of restoring normal sexual life in the observation group was significantly earlier than that of the control group（P〈0.01）;the time of sexual arousal was significantly faster than that of the control group（P〈0.01）;regularity of sexual activity was significantly higher than the control group（P〈0.01）.Conclusion Unprotected perineum delivery combined with continuous epidural analgesia can effectively reduce pain during maternity labor and reduce perineal laceration and lateral rate,so it can improve postoperative life quality.
