摘要
目的探讨血清人附睾蛋白4(HE4)、糖类抗原125(CA125)联合经阴道彩色多普勒超声(TVCDS)诊断早期卵巢癌的临床价值。方法选取经术后病理学确诊的早期卵巢癌患者60例、卵巢良性病变患者40例及体检中心健康女性40例,检测3组女性血清HE4、CA125,同时行TVCDS检测,评估HE4、CA125,联合TVCDS诊断早期卵巢癌的临床价值。结果卵巢癌组的血清HE4、CA125水平显著高于健康组和良性组,差异有统计学意义(P<0.01);卵巢癌组的血管分型主要为3型(80.00%),良性组主要为1型(52.50%)(P<0.01);卵巢癌组的PSV、EDV均显著高于良性组(P<0.01),RI显著低于良性组(P<0.01);HE4鉴别诊断卵巢癌的灵敏度为86.67%、特异度为87.50%;CA125鉴别诊断卵巢癌的灵敏度为78.33%、特异度为77.50%;TVCDS鉴别诊断卵巢癌的灵敏度为83.33%、特异度为80.00%;HE4+CA125+TVCDS鉴别诊断卵巢癌的灵敏度为96.67%、特异度为97.50%。结论HE4+CA125+TVCDS鉴别诊断卵巢癌较各种方法单独应用有显著的临床价值。
Objective To investigate the clinical value of detection of serum human epididymis protein 4 （HE4）, carbohydrate antigen 125 （CA125） combined with transvaginal color Doppler ultrasound （ TVCDS）in diagnosis of early ovarian cancer.Methods Sixty patients with early ovarian cancer （ovarian cancer group） who were diagnosed by pathological examination in our hospital from January 2013 to June 2015 and 40 patients with ovarian benign （benign lesion group） as well as 40 healthy subjects （control group） were enrolled in the study.The serum levels of HE4 and CA125 were detected, moreover,these subjects were examined by TVCDS to evaluate the clinical value of detection of HE 4,CA125 combined with TVCDS in diagnosis of early ovarian cancer .Results The serum levels of HE4 and CA125 in ovarian cancer group were significantly higher than those in control group and benign lesion group （ P 〈0.05）.The vascular type in ovarian cancer group was mainly type 3 （80%）, which in benign lesion group was mainly type 1 （52.50%）, there was a significant difference between two groups（ P 〈0.01）.The levels of PSV and EDV in ovarian cancer group were significantly higher than those in benign lesion group （ P〈0.05）,however, RI in ovarian cancer group was significantly lower than that in benign lesion group （P 〈0.05）.Moreover the sensitivity and specificity of HE 4 in differential diagnosis of ovarian cancer were 86.67%,87.50%,respectively, however ,which of CA125 were 78.33%,77.50%,respectively.Besides the sensitivity and specificity of TVCDS in differential diagnosis of ovarian cancer were 83.33%, 80%, respectively, furthermore, the the sensitivity and specificity of HE4 ＋CA125 ＋TVCDS were 96.67%, 97.50%,respectively.Conclusion The combination detection of HE4 ＋CA125 ＋TVCDS has obvious clinical value in differential diagnosis of ovarian cancer ,as compared with that by the other methods alone.
