摘要
目的:探讨经直肠三维彩色血管能量成像(3DTR-CPA)技术分析前列腺肿瘤的血流特点与肿瘤组织内血管内皮生长因子(VEGF)及增殖指数(Ki-67)的关系。方法:术前应用3DTR-CPA对26例前列腺癌及15例前列腺增生进行血管半定量分型及血管指数(VI)定量分析,术后病理标本应用免疫组织化学技术检测肿瘤内的VEGF及Ki-67表达,进行相关性分析,评价三维能量多普勒成像技术下前列腺肿瘤组织病理学改变。结果:前列腺癌的VEGF、Ki-67表达强度均显著高于前列腺增生(P<0.05),Ⅲ级血管分型的VEGF、Ki-67染色强度高于Ⅰ、Ⅱ级;前列腺癌的血管半定量分型、血管指数VI与VEGF、Ki-67有高度相关性(P<0.05)。结论:经直肠三维彩色血管能量成像(3DTR-CPA)技术血管半定量分型、血管指数(VI)定量分析能为前列腺癌的早期诊断及鉴别诊断提供有价值的信息。
Objective:To investigate the correlation between the expression of VEGF and Ki-67 in tumor and blood flow features of prostatic carcinoma detected by transrectal three-dimensional color power angiography(3DTR-CPA).Methods:Twenty six patients with prostatic carcinoma and 15 patients with benign prostatic hyperplasia were reconstructed with 3DTR-CPA.Half-quantitative and quantitative analysis of vascularity was done with vascularity pattern and vascularity index.The expression of VEGF and Ki-67 was detected by immunohistochemistry.Results:The positive rates of VEGF and Ki-67 expression in the patients of prostatic carcinoma were higher than those in the patients of benign prostatic hyperplasia.The staining intensity of VEGF and Ki-67 in grade Ⅲ was higher than that in grade Ⅰ and grade Ⅱ.The differences were both significant.And there was significant correlation between the expression of VEGF and Ki-67 in tumor and blood flow features of prostatic carcinoma.Conclusion:Vascular pattern and vascularity index of 3DTR-CPA are helpful to diagnoseis and differentiation of prostate diseases.
