摘要
目的探讨薄层液基细胞学(TCT)与阴道镜检查在宫颈癌前病变筛查中的价值。方法行宫颈癌筛查的563例女性分别进行液基细胞学和阴道镜检查,并以病理检查结果为金标准,然后分析TCT检测结果、阴道镜检测结果与病理组织学结果的关系,并对TCT检测、阴道镜检测以及两者联合检测的临床价值分析。结果 563例女性中,两种检测方法中TCT和(或)阴道镜阳性的患者共有178例,178例的病理检查结果为正常或宫颈炎患者34例,CINⅠ期患者54例,CINⅡ期患者49例,CINⅢ期37例,浸润癌患者4例;TCT与病理组织学对宫颈癌筛查的一致性系数为0.755,阴道镜与病理组织学对宫颈癌筛查的一致性系数为0.500,TCT和阴道镜联合检测对宫颈癌诊断的准确性高于单独检测TCT或阴道镜指标的准确性,TCT指标单独检测的准确性高于阴道镜检测的准确性,TCT、阴道镜及两个指标联合检测对宫颈癌诊断的ROC曲线下面积分别为0.840、0.705和0.890。结论 TCT、阴道镜的检测有助于宫颈癌前病变的诊断,联合检测的诊断价值更高。
Objective To investigate the diagnostic value of thinprep cytological test （ TCT ） combined with vaginoscopy in screening of precancerous lesions of cervical cancer .Methods Five hundred and sixty-three female patients underwent screening for cervical cancer by means of TCT or vaginoscopy .Taking the pathological examination results as gold standard,the correlation between the results by TCT or vaginoscopy and those by histopathologic examination was analyzed , moreover , the clinical application value of single TCT detection , single vaginoscopy or combination detection by two methods was evaluated.Results Among 563 woman patients,there were 178 positive cases of TCT detection and （or） vaginoscopy in whom there were 34 normal or cervicitis patients according to pathological examination results , 54 cases of CINⅠstage,49 cases of CIN II stage,37 cases of CIN Ⅲstage,4 cases of invasive carcinoma ,and the consistency coefficient of TCT and pathology for cervical cancer screening was 0.755, which of vaginoscopy and pathology for cervical cancer screening was 0.500.The accuracy of TCT combined with vaginoscopy in diagnosis of cervical cancer was higher than that of TCT or vaginoscopy alone,moreover,which by TCT was higher than that by vaginoscopy .The area under ROC curve in diagnosis of cervical cancer by TCT, vaginoscopy and combination detection of two methods was 0.840,0.705, 0.890, respectively. Conclusion The detection of TCT or vaginoscopy is helpful to the diagnosis of precancerous lesions of cervical cancer , moreover , the combination detection of TCT with vaginoscopy has higher diagnostic value .
