摘要
目的 :探讨 P2 7、Ki- 6 7抗原在人脑星形胶质细胞肿瘤中的表达及预后意义。方法 :原发性星形细胞肿瘤 73例采用 S- P免疫组化方法检测手术切除标本石蜡块的 P2 7抗原和 Ki- 6 7抗原 ,并分别算得两者标记指数。使用 COX比例风险模型分析预后。结果 :单因素分析显示患者年龄、手术治疗方式、术前 KPS评分、组织学分级、Ki- 6 7L I、P2 7L I、术后放疗等因素与患者预后有关 (logrank P<0 .0 1)。多因素分析结果显示年龄、P2 7L I、Ki- 6 7L I、术前KPS评分、组织学分级均为独立的预后因素。结论 :组织学分级、患者的年龄、P2 7L I、Ki- 6 7L I、术前 KPS评分均是独立的预后影响指标。 P2 7L I随着各病理级别增高而下降 ;在同一病理级别中 ,P2 7L I不同 ,其预后有显著性差异。
Objective : To explore the prognostic factors for patients with astrocytic tumors and to determine the value of P27 labeling index obtained using P27 monoclonal antibody in predicting survival. Methods: Data of 73 patients with astrocytic tumors were collected. P27 and Ki-67 was determined with immunohistochemistry using monoclonal antibody. Cox's proportional-hazards model was used to ascertain a simultaneous effect of outcome-related variables on survival . Results: Univariate analysis showed that patient age, KPS score, histologic grade, Ki-67 LI ,P27 LI and radiation were significant factors for survival. And Multivariate survival analysis showed that patient age, histologicgrade, Ki-67 LI ,P27 LI and KPS score were independent , tatistically significant prognostic factors for patients with astrocytic tumors. Conclusion: Histologic grade and age, preoperative KPS , P27LI, Ki-67LI are all independent prognosis factors .In the same histologic grade there is a significant difference in patient's prognosis due to the different P27LI.
