摘要
目的探讨免疫抑制剂联合中药补肾益髓方治疗儿童再生障碍性贫血的临床效果和对外周血象免疫因子的影响。方法选取2014年1月~2015年5月在滨州医学院附属医院治疗的60例再生障碍性贫血患儿,随机分为治疗组和对照组,各30例。对照组给予免疫抑制剂治疗,治疗组在其基础上联合中药补肾益髓方治疗。6个月后,对比两组的临床效果及患儿的外周血象、淋巴细胞亚群(CD3+、CD3+CD4+、CD3+CD8+、CD3+CD56+细胞及CD4+/CD8+比值)的水平。结果治疗组总有效率为86.67%,高于对照组的70.00%,差异有统计学意义(P<0.05)。治疗后,两组患儿的血红蛋白、白细胞、血小板和淋巴细胞亚群水平均有所提高,两组患儿淋巴细胞亚群均恢复到正常水平,与正常健康值比较差异无统计学意义(P>0.05);且治疗后治疗组各指标水平高于对照组,差异有统计学意义(P<0.05)。结论免疫抑制剂联合中药补肾益髓方治疗儿童再生障碍性贫血较单纯应用免疫抑制剂具有更好的临床疗效,能够有效调节患儿机体的免疫反应,促进骨髓造血功能,值得临床推广应用。
Objective To explore the clinical effect of immunosuppressor combined with Bushen Yisui decoction in the treatment of children with aplastic anemia and its influence on peripheral hemogram immune factors.Methods Sixty children with aplastic anemia treated in Affiliated Hospital of Binzhou Medical University from January 2014 to May2015 were selected and randomly divided into treatment group and control group,wtih 30 cases in each group.In the control group,immunosuppressor therapy was used,while in the treatment group,Bushen Yisui decoction was added.After6-months treatment,the clinical efficacy,the levels of peripheral hemogram and lymphocyte subsets（CD3＋,CD3＋CD4＋,CD3＋CD8＋,CD3＋CD56＋and CD4＋/CD8＋）were compared between the two groups.Results The total effective rate was86.67% in the treatment group and 70.00% in the control group,and the difference was statistically significant（P〈0.05）.The levels of Hb,WBC,PLT and lymphocyte subsets（CD3＋,CD3＋CD4＋,CD3＋CD8＋,CD3＋CD56＋and CD4＋/CD8＋）were all increased;The levels of CD3＋,CD3＋CD4＋,CD3＋CD8＋,CD3＋CD56＋and CD4＋/CD8＋in both groups were recovered to be normal and had no statistical difference compared with that in the healthy individuals（P〈0.05）;the levels of all indexs in the treatment group after treatment were higher than those in the control group,and the differences were statistically significant（P 0.05）.Conclusion The effect of combination of immunosuppressant and traditional Chinese medicine Bushen Yisui decoction in children with aplastic anemia is better than immunosuppressor alone.It can effectively improve the immune response in children and promote the hematopoietic function,and it′s worthy of clinical application and promotion.
