颅脑超声对新生儿颅内出血的临床诊断价值分析 被引量:7

摘要 目的研究颅脑超声对新生儿颅内出血患儿的临床诊断价值并进行分析。方法回顾性分析从2014年3月-2016年2月,于我院接受诊治的疑有颅内出血新生儿87例,所有新生儿均进行颅脑超声检查,根据胎儿是否早产进行分组,并与螺旋CT检查进行对比分析。结果经颅脑超声检出77例颅内出血患者,后经金标准确诊颅内出血患儿共87例,颅脑超声的检出率为88.51%(77/87)。早产儿颅内出血人数占比为71.43%(55/77),显著高于足月儿的28.57%(22/77),差异有统计学意义(P〈0.05)。早产患儿以及足月患儿主要为脑室出血,其次是脑实质出血,硬膜下出血较为少见,二者相比差异无统计学意义(P〉0.05)。颅内出血患儿分级主要为Ⅰ级,其次为Ⅱ级,Ⅲ级以及Ⅳ级较为少见,Ⅰ级及Ⅱ级的占比明显高于Ⅲ级及Ⅳ级,差异有统计学意义(P〈0.05)。结论颅脑超声对新生儿颅内出血患儿的临床诊断价值较高,可作为筛查颅内出血的重要方法。 Objective To study the clinical diagnostic value of brain ultrasound to neonatal intracranial hemorrhage,and to analyze the value. Methods 87 cases of neonatal intracranial hemorrhage cured in our hospital from March 2014 to February 2016 were retrospectively analyzed.All the neonatus were treated with brain ultrasound.They were divided according to premature or not.And carries on the contrast analysis with the spiral CT examination. Results 77 cases of intracranial hemorrhage were detected by transcranial ultrasound.And,87 cases were detected as intracranial hemorrhage by the gold standard diagnosis.The detection rate of brain ultrasound was 88.51%(77/87).The proportion of intracranial hemorrhage in premature infants was 71.43%(55/77),which was significantly higher than that of full-term infants 28.57%(22/77),and the difference was statistically significant(P〈0.05).The main bleeding locations of preterm infants and term children was ventricular hemorrhage,followed by cerebral hemorrhage,subdural hemorrhage was rare,compared to the two,the difference was not statistically significant(P〉0.05).Classification of intracranial hemorrhage in patients was grade Ⅰ,followed by Ⅱ, Ⅲ and IV were rare,grade Ⅰ and Ⅱ were significantly higher than grade Ⅲ and Ⅳ,the difference was statistically significant(P〈0.05). Conclusion Brain ultrasound has high clinical value of in the diagnosis of neonatal intracranial hemorrhage.It can be used as an important method of screening for intracranial hemorrhage.
