应用超声心动图评价房颤对左室射血分数正常的老年心力衰竭患者左房功能的影响 被引量:16

摘要 目的应用超声心动图定量评估房颤对左室射血分数正常的老年心力衰竭(HFNEF)患者左房功能的影响。方法收集有心力衰竭症状的老年患者60例,左室射血分数(LVEF)均正常,其中伴有房颤的30例(房颤组),不伴房颤的30例(非房颤组)。另选取30例健康者设为对照组。应用超声心动图诊断仪测量其左房前后径(LAAPD)、横径(LAMLD)、上下径(LASID)、左房最大容积(Vmax)、最小容积(Vmin)及左房射血分数(LAEF),计算其左房射血力(LAF)及Tei指数等传统指标,同时测量左房各壁心肌应变率(SR)。比较各组指标。结果与传统指标相比,左房各壁SR随个体变化不大,在反映左房功能上具有良好一致性。与对照组比较,HFNEF患者各传统指标及左房SR均有显著差异(P<0.05)。当合并房颤时,LAEF及左房各壁SR进一步降低(P<0.05),而LAF呈增高趋势,与病理生理改变不符;Tei指数无显著性改变。结论针对局限性左房功能的评估价值,左房SR明显优于LAEF、LAF、Tei指数。
