摘要
目的明确福建地区乙肝病毒(hepatitis B virus,HBV)基因型的分布情况及其特征。方法通过巢式PCR法分片段扩增慢性乙肝患者HBV全基因组序列,构建系统发育树确定福建地区乙型肝炎病毒主要基因型、血清型;同时结合HBV-DNA载量、血清标志物等检测结果分析福建地区乙肝基因型分布特征。结果从147例血清样本中成功扩增126例HBV完整基因序列;系统进化树分析表明,126例血清样本中B基因型78例(61.90%),C基因型42例(33.33%),B/C混合型6例(4.76%),未检测到其他基因型。血清型以adw2和adrq+为主,分别占63.49%和36.51%;不同基因型的分布与患者年龄、性别、HBV-DNA载量、所在地区均无相关性(P>0.05),而与患者血清标志物HBe Ag的阳性率、HBV血清型具有一定的相关性(P<0.05)。结论福建地区乙肝基因型主要以B型和C型为主,另见少量B/C混合型。B、C不同基因型的分布与患者血清标志物HBe Ag的阳性率及HBV的血清型具有一定相关性。
Objective To determine the distribution of hepatitis B virus（HBV）genotypes and its characteristics inFujian area. Methods Using the method of nested PCR amplification HBV gene sequences and phylogenetic tree wasconstructed to determine the main genotypes and serotype of hepatitis B; combined with HBV- DNA loads,serum markerdetection results was used to analyze HBV genotype distribution. Results A total of 126 HBV genetic sequence weresuccessfully amplified from 147 samples. Genotypes C and B were the majority genotypes of HBV with 78（61.90%） cases ofgenotype B, 42（33.33%） cases of genotype C, 6（4.76%） cases of mixed type B/C. adw2 and adrq＋ were the leading subtypes ofHBV in these area, taking up 63.49% and 36.51%, respectively. The distribution of different genotype was no correlation withpatients＇ age, gender, the concentration of HBV- DNA, region, and was certain relevance with the patient＇s serum markerHBe Ag positive rate, serum HBV, with significantly statistical difference（P〈0.05）. Conclusion HBV genotypes in Fujian isgiven priority to with B type and C type, and a small amount of B/C blended. Gene distribution has a certain relationship withthe patient＇s serum marker HBe Ag positive rate and HBV serum type.
