摘要
目的:探讨低温等离子与CO_2激光微创治疗早期声门型喉癌的疗效及安全性。方法:收取2014年5月至2016年4月间我院收治的早期声门型喉癌患者79例作为研究对象进行回顾性分析,按照治疗方式的不同将其分为观察组42例及对照组37例。观察组行低温等离子射频消融治疗,对照组行CO_2激光微创治疗。比较两组患者手术时间、术中出血量及疼痛程度评分、嗓音学指标、声带形态学评分、术后生活质量以及并发症发生情况。结果:观察组手术时间明显短于对照组(P<0.05);两组患者术中出血量及术后疼痛程度评分无显著差异(P>0.05)。治疗后各声学指标较治疗前显著下降,差异均有统计学意义(P<0.05)。观察组各指标均低于对照组,差异有统计学意义(P<0.05)。手术后两组患者声带形态学评分均较治疗前显著下降,术后1个月时观察组评分显著低于对照组(P<0.05)。观察组术后生活质量各维度均明显优于对照组,差异有统计学意义(P<0.05)。所有患者术后均未发现严重并发症及复发。结论:低温等离子与CO_2激光微创治疗早期声门型喉癌均具有良好安全性,但低温等离子患者术后恢复情况优于CO_2激光治疗,值得临床推广应用。
Objective: To explore the efficacy and safety ofmicrotherm plasma radio frequency and CO2 laser in treatment of early glottis squamous cell carcinoma. Methods: 79 patients with early glottis squamous cell carcinoma accepted in our hospital from May 2014 to April 2016 were retrospectively analyzed and divided into observation group with 42 patients and control group with 37 patients according to different treatment methods. Patients in observation group were given microtherm plasma radio frequency treatment, and in control group were given CO2 laser treatment. The operation time, blood loss, pain grade, voice acoustic, vocal shape, life quality and complications of two groups were observed and compared. Results: The operation time of observation group was obviously shorter than that of control group with statistically significance (P〈0.05); the blood loss and pain grade of two groups had no statistically difference (P〉0.05). The indexes of voice acoustic were decreased after treatment with statistically significance (P〈0.05) and they were obviously lower in observation group than control group (P〈0.05). The scores of vocal shape of two groups were lower after treatment than before, and one month after operation these scores were lower in observation group than control group with statistically significance (P〈0.05). The life quality of observation group after treatment was lower than that of control group (P〈0.05). There were no obvious complications or recurrence of two groups. Conclusions: Microtherm plasma radio frequency has similar safety with CO2 laser in treatment of early glottis squamous cell carcinoma, but has better recovery, which is worth clinical applications.
