摘要
目的研究一期前后路联合减压融合内固定术治疗重型颈椎后纵韧带骨化症的临床效果。方法选择2013年5月至2014年11月42例重型颈椎后纵韧带骨化症患者作为研究对象，随机分为两组，对照组18例行颈后路减压融合内固定术，观察组24例行颈前后路联合减压融合内固定术。比较两组手术效果和并发症发生情况。结果观察组术后JOA评分改善（IR）优良率为95.83%，显著高于对照组（77.78%），差异有统计学意义（P〈0.05）；观察组术后并发症发生率略低于对照组，差异未见统计学意义（P〉0.05）。结论一期前后路联合减压内固定术治疗重型颈椎后纵韧带骨化症效果显著，并发症较低，临床应用价值较高。
ObjectiveTo study the clinical effect of one-staged anterior and posterior approach combined decompression combined with internal fixation on ossification of the posterior longitudinal ligament.
MethodsForty-two patients with severe ossification of posterior longitudinal ligament from May 2013 to November 2014 were selected as the research objects and were randomly divided into two groups, with 18 cases in the control group and 24 cases in the observation group. The control group was treated by cervical posterior approach decompression combined with internal fixation, while the observation group was treated by anterior and posterior approach combined decompression combined with internal fixation. The operation effect and occurrence of complications were compared between the two groups.ResultsThe improvement rate （IR） of JOA score of the observation group （95.83%） was significantly higher than that of the control group （77.78%, P〈0.05）. The incidence of postoperative complications in the observation group was slightly lower than that in the control group （P〉0.05）.ConclusionsThe effect of one-staged anterior and posterior approach combined decompression combined with internal fixation on ossification of the posterior longitudinal ligament is remarkable, with low complications and high clinical application value.
