摘要 目的:探讨脓毒症大鼠心肌高迁移率族蛋白B1(HMGB1)在脓毒症心肌损害中的表达和作用。方法:48只清洁级健康雄性Wistar大鼠随机分成正常对照组(Normal组)、假手术组(Sham组)、脓毒症模型组(CLP组),各16只。于手术后24 h、48 h分别检测HMGB1、肿瘤坏死因子(TNF)-α、白细胞介素(IL)-6、肌钙蛋白(Tn)I浓度,光镜观察心肌病理变化。结果:与Normal及Sham组比较,CLP组在24 h、48 h HMGB1、TNF-α、IL-6及Tn I水平均显著升高,差异有统计学意义(P<0.01);CLP组术后48 h较24 h HMGB1增加值与TNF-α、IL-6及Tn I增加值间分别呈显著正相关(r=0.886,P<0.05;r=0.943,P<0.05;r=0.829,P<0.05)。光镜下CLP组可见心肌损伤。结论:HMGB1与脓毒症心肌损害密切相关,其介导的炎性反应在心肌损害的发生发展过程中起着重要的作用。 Objective To study the expression of the high mobility group box - 1 (HMGB1) in myocardial damage of rats with sepsis and related mechanism. Method A total of 48 male Wistar rats of clean grade were randomly divided to the Normal group(Normal), the sham -operation group (Sham) and the cecal ligation puncture group (CLP), 16 in each group. Concentrations of HMGB1 ,tumor necrosis factoralpha (TNF) -α, interleukin (IL) -6 ,and troponin (Tn) I were detected in each group at 24 h and 48 h after operation. Pathological changes of cardiac muscle were observed under light microscope. Results Concentrations of HMGB1, TNF -a, IL -6 and TnI at 24 h and 48 h after operation were significantly higher in the CLP group than in the Normal group and the Sham group (P 〈 0. 01 ). Concentration Increments of HMGB1 correlated posi- tively with TNF - a, IL - 6 and TnI at 48 h versus 24 h after operation ( r = 0. 886, P 〈 0.05 ;r = 0. 943, P 〈 0. 05 ; r = 0. 829,P 〈0 . 05 ). Myocardial injury occurred in the CLP under light microscope. Conclusion HMGB1 is closely related to myocardial damage in sepsis and the inflammatory response induced by HMGB1 could play an important role in development and progression of myocardial damage.
