摘要
目的探究急性白血病合并感染患者的C反应蛋白（CRP）及D-二聚体（D-D）水平变化及临床诊断价值。方法将我院血液科收治的68例急性白血病合并感染患者作为研究对象，并选择23例同期进行健康体检的志愿者作为对照，对比两组入选者的CRP及D-D水平。按是否并发DIC（弥散性血管内凝血）将感染组分为单纯感染组（52例）和感染并发DIC组（16例）两个亚组，比较两组患者的CRP和D-D水平。根据CRP水平的高低将感染组分为三个组，对比三组之间的肛D水平及DIC发生的比例。结果感染组的CRP及D-D水平均远远高于健康对照组（P〈0．05）。而在感染组中，感染并发DIC组的CRP及D-D水平均显著高于单纯感染组（P〈0．05）。随着CRP水平的不断升高，感染组患者的D-D水平及DIC所占比重也逐渐上升，组间比较差异显著（P〈0．05），CRP水平与D-D水平、DIC发生率呈正相关。结论急性白血病患者并发感染及DIC后，其CRP及D-D水平上升明显。可将CRP、D-D作为检测指标，为临床及早诊断患者是否并发感染和DIC提供重要参考。
Objective To research the change and clinical diagnostic value analysis of CRP and D-dimer in patients with acute leukemia complicated with infection. Methods 68 cases of acute leukemia patients complicated with infection in our hospital were selected as the research subjects, and 23 healthy volunteers were selected as control group, compared the CRP and D-D levels between the two groups. According to whether the concurrent DIC （disseminated intravaseular coagulation）, the infection group was divided into the simple infection group （52 cases） and the infected DIC group （16 cases）, compared the CRP and D-D levels in two subgroups. According to the level of CRP, the infection group was divided into three groups, and compared the ratio of the D-D level and the incidence of DIC between the three groups. Results The levels of CRP and D-D in the infection group were much higher than those in the healthy control group （P（0.05）. In the infection group, the CRP and D-D levels of the infected DIC group were significantly higher than those of the simple infection group （P（0.05）. With the rising of CRP level, the D-D level and the proportion of DIC in infected patients gradually increased, there were significant differences between the groups （P〈0.05）, the CRP level was positively correlated with the D-D level and the incidence of DIC. Conclusion After infection and DIC in patients with acute leukemia, their CRP and D-D levels in- creased significantly. D-D and CRP can be used as indicators to provide important reference for clinical diagnosis and early diagnosis of a cute leukemia patients with infection and DIC.
