摘要
目的观察初诊2型糖尿病合并周围神经病变患者氧化应激产物丙二醛(MDA)及超氧化物歧化酶(SOD)和炎性细胞因子肿瘤坏死因子α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-1β(IL-1β)的变化,探讨氧化应激和炎性反应与该病的相关性。方法选取初诊2型糖尿病合并周围神经病变患者70例作为周围神经病变组,无周围神经病变的初诊2型糖尿病患者70例作为糖尿病组,体检正常者70例作为正常组。ELISA法分别测定3组TNF-α、IL-6、IL-1β水平,黄嘌呤氧化酶法测定SOD活性,硫代巴比妥酸法测定MDA水平。采用多因素相关分析方法,分析氧化应激及炎性反应与初诊2型糖尿病合并周围神经病变之间的关系。结果周围神经病变组和糖尿病组FPG、2h PG、Hb A1c、TNF-α、IL-6、IL-1β、MDA、SOD水平均明显高于正常组(P均<0.05)。多因素相关分析显示TNF-α、IL-6、MDA对初诊2型糖尿病是否合并周围神经病变均有独立作用,且MDA与SOD呈负相关。结论氧化应激和免疫炎性反应参与了2型糖尿病周围神经病变的发生及发展。
Objective It is to observe the changes of malondialdehyde( MDA),antioxidant enzyme superoxide dismutase( SOD),cytokine TNF-α,IL-6,IL-1β in patients with newly diagnosed type 2 diabetes mellitus( DM) complicating with peripheral neuropathy( PN),and to explore the correlation of oxidative stress and inflammatory reaction with this disease.Methods Seventy patients with newly diagnosed DM with PN were selected as PN group,70 ones without PN as DM group,70 healthy people as normal group. The levels of TNF-α,IL-6,IL-1β in the three groups were determined by ELISA,the activity of SOD was detected by xanthine oxidase method,the level of MDA was determined by glucosinolates barbituric acid method.Multiple factor correlation analysis method was used to analyze the correlation of oxidative stress and inflammatory reaction with DM complicating with PN. Results The levels of FPG,2h PG,Hb A1 c,TNF-α,IL-6,IL-1β,MDA,SOD in DM group and PN group were significantly higher than those in normal control group( P〈0. 01). Multi-factor correlation analysis showed that TNF-α,IL-6,MDA had independent role in the diagnosis of newly diagnosed type 2 diabetes whether complicating with PN,and MDA was negatively related with SOD. Conclusion Oxidative stress and immune inflammatory reaction involved in the development of type 2 diabetes with peripheral neuropathy.
