子痫前期患者血清D-二聚体的水平及其与血脂水平的相关性 被引量:16

摘要 目的探讨子痫前期(PE)患者血清D-二聚体的水平及其与血脂水平的相关性。方法选取2014年12月至2015年12月期间首都医科大学附属北京友谊医院收治的80例PE孕妇作为PE组,并根据PE病情分为重度PE组(n=42)和轻度PE组(n=38);其中重度PE组又根据发病时间分为早发型重度PE组(n=18)和晚发型重度PE组(n=24)。选择2014年12月至2015年12月期间待产的80例正常妊娠的孕妇作为正常对照组。对比观察各组血压、血脂、血D-二聚体、血尿素氮(BUN)、肌酐(Scr)、尿酸(UA)等指标水平。结果 PE组收缩压、舒张压高于正常妊娠组(P<0.05),PE组D-二聚体、TG、TC、LDL-C水平高于正常妊娠组(P<0.05),PE组HDL-C水平低于正常妊娠组(P<0.05);重度PE组收缩压、舒张压高于轻度PE组(P<0.05),重度PE组D-二聚体、TG、TC、LDL-C水平高于轻度PE组(P<0.05),HDL-C水平低于轻度PE组(P<0.05)。PE组BUN、Scr、UA水平均高于正常妊娠组(P<0.05)重度PE组BUN、Scr、UA均高于轻度PE组(P<0.05)。早发型重度PE组D-二聚体水平(2.02±0.61μg/ml)高于晚发型重度PE组(1.64±0.52μg/ml)差异有统计学意义(t=3.27,P=0.014,P<0.05)。PE患者血清D-二聚体水平与收缩压、舒张压均呈正相关(r=0.467,0.559,P=0.000,0.000),与TG、TC、LDL-C均呈正相关(r=0.627,0.615,0.572,P=0.000,0.000,0.000),与HDL-C呈负相关(r=-0.591,P=0.000),与BUN、Scr、UA均呈正相关(r=0.621,0.582,0.594,P=0.000,0.000,0.000)。结论 PE患者血清中D-二聚体水平高于正常妊娠孕妇且D-二聚体水平与血脂、血压、肾功能等指标相关,D-二聚体水平与PE患者的病情严重程度及发病时间有关。 Objective To study blood D- dimer level of preeclampsia patients and its correlation with blood lipid level.Methods From December 2014 to December 2015,80 PE pregnant women were selected as PE group,and according to the PE condition they were divided into severe PE group(n =42) and mild PE group(n =38).The severe PE group according to the onset time were divided into early onset severe PE group(n = 18) and late onset severe PE group(n =24).During the same period,80 cases of normal pregnant women were selected as control group.The levels of blood pressure,blood lipid,blood D- two,blood urea nitrogen(BUN),creatinine(Scr) and uric acid(UA) were observed and compared.Results In PE group,systolic blood pressure and diastolic blood pressure were higher than those of the normal pregnancy group(P〈 0.05).TG,D- dimers,TC,LDL- C level in PE group were higher than those in the normal pregnancy group(P 〈0.05).The level of HDL- C in PE group was lower than that of normal pregnancy group(P 〈0.05).Systolic and diastolic blood pressure in severe PE group was higher than that of mild PE group(P 〈0.05).D- dimer,TG,TC and LDL- C in severe PE group were higher than those of mild PE group(P 〈0.05),HDL- C level was lower than that of the mild PE group(P〈 0.05).BUN,Scr and UA in PE group were higher than those in normal pregnancy group(P〈 0.05),BUN,Scr and UA in severe PE group were higher than those in mild PE group(P〈 0.05).D- dimer level(2.02 ±0.61 μg/ ml) in the early onset of severe PE group was higher than that of late onset severe PE group(1.64 ± 0.52 μg/ml),the difference was statistically significant(t =3.27,P =0.014,P 〈0.05).Serum D- dimer and systolic blood pressure and diastolic blood pressure were positively correlated(r =0.467,0.559,P =0.000,0),which was positively correlated with TG,TC,LDL- C(r =0.627,P =0.000,0.615,0.572,0,0),and negatively correlated with HDL- C(r =-0.591,P =0.000),and positively correlated with BUN,Scr,UA(r =0.621,P =0.000,0.582,0.594,0,0).Conclusion Serum D- dimer levels of PE patients are higher than those of normal pregnant women,and D- dimer level is related to blood lipid,blood pressure,kidney and other related indicators,and D- dimer level of patients related with PE illness severity and duration of illness.
