摘要
目的研究复方苦参注射液联合自制消臌散治疗恶性腹腔积液的临床效果。方法选取2014年1月至2016年1月中山医院青浦分院中医肿瘤科病房收治气滞血瘀,湿热内蕴型恶性腹腔积液病例90例,采用随机、对照的临床研究方法,将这些患者分为消臌散组(n=30)、联合用药组(复方苦参注射液联合消臌散,n=30)、对照组(西药利尿剂,n=30)三组,分别采取相应药物治疗。对三组患者的临床疗效、生活质量及不良反应发生情况进行统计分析。结果中药联合治疗组、消臌散组患者治疗的总有效率93.3%、73.3%均显著高于对照组50.0%(P<0.05);而中药联合治疗组患者治疗的总有效率又显著高于消臌散组(P<0.05)。中药联合治疗组、消臌散组患者的生活质量改善率均显著高于对照组(P<0.05),KPS评分均显著高于对照组(P<0.05),稳定率、降低率均显著低于对照组(P<0.05),而中药联合治疗组患者的生活质量改善率又显著高于消臌散组(P<0.05),KPS评分显著高于消臌散组(P<0.05),稳定率、降低率均显著低于消臌散组(P<0.05)。中药联合治疗组、消臌散组患者的不良反应发生率(13.3%、10.0%)均显著低于对照组(33.3%),而中药联合治疗组、消臌散组患者的不良反应率无显著差异(P>0.05)。结论气滞血瘀、湿热内蕴型恶性腹腔积液采用复方苦参注射液联合自制消臌散治疗安全有效,能显著提高患者生活质量。
Objective To study the clinical effect of compound matrine injection combined with homemade xiaogu scattered in the treatment of malignant ascites.Methods 90 cases of patients with Qi stagnation,damp type malignant ascites in TCM oncology wards of Qingpu Branch of Zhongshan Hospital from January 2014 to January 2016 were selected,these patients were divided into xiaogu powder group（n = 30）,combined treatment group（Compound Sophora Injection joint Xiaoguruanjian scattered,n = 30）,control group（Western diuretics,n = 30）.They were given corresponding drug treatment,respectively.The clinical efficacy,quality of life and adverse events of the three groups were statistically analyzed.Results The total treatment efficiency of the combined treatment group 93.3%（28 /30）,xiaogu powder group,73.3%（22 /30） were significantly higher than that of the control group 50.0%（15 /30）（P 〈0.05）,while the total treatment efficiency of the combined treatment group was higher than that of the xiaogu powder group（P 〈0.05）.The combination of traditional Chinese medicine treatment group,Xiaogu San groups life quality improvement rate were significantly higher than that of the control group（P 〈0.05）.The KPS scores were significantly higher than that of the control group（P〈 0.05）,the stable rate and reduction rate were significantly lower than those of the control group（P 〈0.05）,and the quality of life of traditional Chinese medicine combined with the improvement rate in the treatment group was significantly higher than that of the Xiaogu powder group（P〈 0.05）.The stable rate and the decrease rate were significantly lower than the Xiaogu powder group（P〈 0.05）.The KPS score was significantly higher than that of the Xiaogu powder group（P〈 0.05）.The combination of traditional Chinese medicine treatment group,Xiaogu San group of patients with adverse reaction rate（13.3%,10%） were significantly lower than that of the control group（33.3%）,and the difference of adverse reaction between the combination of traditional Chinese medicine treatment group,Xiaogu San group was not significant（P 〉0.05）.Conclusion The clinical effect of compound matrine injection combined with homemade xiaogu scattered in the treatment of Qi stagnation,damp type malignant ascites is safe and effective.It can significantly improve the life quality of patients.
