摘要
目的探讨空腹胰岛素(FPI)水平对老年男性2型糖尿病患者腰椎骨密度(BMD)的影响情况。方法选取2015年1月至2015年12月应用定量CT(QCT)测定的30例2型老年男性糖尿病患者及12例血糖正常老年男性腰椎骨密度及空腹血浆胰岛素水平,并分别设定其为观察组和对照组。结果①观察组FPI水平高于对照组,差异有统计学意义(P〈0.05)。②观察组腰椎BMD低于对照组,差异有统计学意义(P〈0.05)。③观察组骨质疏松症(OP)的患病率高于对照组,差异未见统计学意义(P〉0.05)。④根据2型糖尿病患者使用胰岛素治疗情况,分为A组(胰岛素使用1年以上)、B组(胰岛素使用1年以下)、C组(口服药物组),三组腰椎BMD值A组高于B、C组,差异有统计学意义(P〈0.05);B组高于C组,差异有统计学意义(P〈0.05)。结论老年男性2型糖尿病患者BMD低于正常老年男性,较高的胰岛素水平可能对BMD有保护作用。
Objective To investigate the effects of fasting plasma insulin (FPI) level on bone mineral density(BMD) of lumbar spine in elderly male patients with type 2 diabetes mellitus (T2DM). Methods From January 2015 to December 2015, 30 cases of T2DM and 12 cases of normal blood glu- cose in elderly mail patients were selected. All patients got the data of lumbar BMD by quantitative CT (QCT) and FPI level, and were respectively divided into the observation group and control group. Results ①The FPI level was significantly different between the two groups (P 〈 0. 05 ). ②The lumbar BMD of observation group was lower than that of control group (P 〈 0. 05 ). ③The prevalence of osteopo-rosis (OP) in observation group had no significant difference compared with the control group (P 〉0. 05 ). ④The observation group were divided into three groups according to the time of insulin in therapy of T2DM, group A(more than 1 year), group B (less than 1 year) and group C(oral drug). The BMD of lumbar spine of group A was higher than that of group B and C ( P 〈 0. 05 ), and the lumbar spine BMD of group B was higher than that of group C (P 〈0. 05 ). Conclusions BMD in elderly male patients with T2DM are lower than those of normal elderly men, and the high level of insulin may have a protective effect in BMD.
