摘要
目的观察对比改良腹腔镜下完全腹膜外疝修补术(TEP)和开放疝环填充式无张力疝修补术(Rutkow术式)近远期疗效。方法选取咸阳市第一人民医院收治的腹股沟疝患者60例作为研究对象,按照随机数字表法分为改良TEP组和Rutkow术式组,每组患者30例。观察2组患者住院时间、术中出血量、手术时间以及术后下床活动时间、并发症发生率以及复发情况。结果改良TEP组住院时间、术中出血量、术后下床活动时间显著少于Rutkow术式组(t=12.015、5.883、12.407,P<0.05),2组手术时间比较差异无统计学意义(t=0.775,P>0.05);改良TEP组并发症发生率3.33%显著低于Rutkow术式组20.00%(χ2=4.043,P<0.05);改良TEP组复发率3.33%显著低于Rutkow术式组23.33%(P<0.05)。结论改良TEP与Rutkow术式均能治疗腹股沟疝,但改良TEP近远期疗效优于Rutkow术式。
Objective To observe and compare the short-term and long-term effect of modified laparoscopic total extraperitoneal inguinal hernia repair （TEP） and open mesh-plug tension-free hernia repair （ Rutkow surgery）. Methods 60 cases of patients with inguinal hernia treated in the First People＇s Hospital of Xianyang were selected and divided into modified TEP group and Rutkow surgery group according to the random digital table method, with 30 cases in each group. The hospital stay, blood loss during operation, operation time, postoperative bed time, incidence rates of complications and recurrence in the two groups were observed. Results The hospital stay, blood loss during operation, operation time, postoperative bed time in the modified TEP group were significantly less than those in the Rutkow surgery group （t = 12. 015,5. 883,12. 407, P 〈 0. 05 ）. There was no statistically significant difference in the operation time between the two groups （ t = 0. 775, P 〉 0.05 ）. The incidence rate of complications in the modified TEP group was 3.33% , which was significantly lower than that of 20.00% in the Rutkow surgery group （X^2 =4. 043,P 〈0.05）. The rate of recurrence in the modified TEP group was 3.33%, which was significantly lower than that of 23.33% in the Rutkow surgery group （ P 〈 0.05 ）. Conclusion Both modified TEP and Rutkow surgery can be used to treat the inguinal hernia, but the short-term and long-term effect of modified TEP is better than that of Rutkow surgery.
