摘要
目的 :对比分析不同比重布比卡因腰硬联合麻醉应用于老年下肢骨科手术的麻醉效果。方法 :选取我院行下肢骨科手术的老年患者96例,随机分为A、B、C 3组各32例。A、B和C组分别行重、等和轻比重布比卡因麻醉,比较3组患者的麻醉效果。结果 :3组患者达最高阻滞平面时间比较差异无统计学意义(P>0.05);患者感觉阻滞起效时间依次为A组<B组<C组(P<0.05)。A、B组麻醉后5 min MAP、HR显著下降(P<0.05),至麻醉后10 min后在正常范围内波动;B、C组麻醉后MAP、HR在正常范围内波动;3组患者术中均无明显呼吸抑制发生,SpO_2均在正常范围内波动。结论 :等比重布比卡因腰硬联合麻醉应用于老年下肢骨科手术麻醉效果满意,易调控,且对血流动力学影响较小,值得临床进一步实践。
Objective： To compare the effects of the spinal-epidural anesthesia combined with different specific gravity of bupivacaine on the elderly patients with lower limb orthopedic surgery. Methods： Ninety-six elderly patients with lower limb surgery were selected and randomly divided into group A, B and C with 32 cases each. Group A, B and C underwent a heavy, equal and light specific gravity of bupivacaine anesthesia, respectively and their anesthetic effects were compared. Results： There was no statistical significance in the time for reaching the highest block plane in three groups（P〉0.05）. The time for the onset of sensory block was less in group A than group B and C in turn（P〈0.05）. The time for reaching the highest block plane was significantly less in group A than group B and C（P〈0.05） while slightly less in group B than group C（P〉0.05）. MAP and HR were significantly decreased in group A and B at 5 min after anesthesia（P〈0.05） and then fluctuated in the normal range at 10 min after anesthesia while they were fluctuated in the normal range in group B and C after anesthesia. There was no significant respiratory depression occurred and SpO_2 was fluctuated in the normal range in three groups. Conclusion： The spinal-epidural anesthesia combined with equal specific gravity of bupivacaine possesses satisfactory efficacy and less effect on hemodynamics and is easily controllable and worthy of clinical further practice.
