摘要
目的:探讨心脏彩超对高血压左室肥厚伴左心衰竭的诊断价值。方法:选择78例高血压左室肥厚伴左心衰竭者(心衰组),其中NYHA Ⅱ级组38例,Ⅲ级组40例;健康体检者(健康对照组)120例作为研究对象。所有对象均行心脏彩超检查,比较各组左室舒张末期内径(LVEDd)、左房内径(LAD)、左室射血分数(LVEF)、二尖瓣口舒张早期最大血流速度/二尖瓣舒张早期最大运动速度(E/Ea)的差异。结果:与健康对照组比较,观察组LVEDd[(45.63±5.32)mm比(57.24±11.16)mm]、LAD[(33.25±6.54)mm比(50.53±10.92)mm]、E/Ea[(8.01±4.21)比(14.21±3.52)]均显著升高,LVEF[(69.73±11.43)%比(54.36±10.51)%]显著降低,P均<0.05。与Ⅱ级组比较,Ⅲ级组LVEDd[(57.12±9.53)mm比(66.20±7.35)mm]、LAD[(46.36±6.94)mm比(53.14±7.21)mm]、E/Ea[(12.89±3.02)比(15.17±3.53)]均显著升高,LVEF[(46.31±10.23)%比(38.75±8.61)%]显著降低,P<0.05或<0.01。结论:心脏彩超对于高血压左室肥厚伴左心衰竭具有一定的诊断价值,可用于评估此类患者的病情严重程度,值得临床推广及运用。
Objective：To explore diagnostic value of ultrasonic cardiogram（UCG）for hypertensive left ventricular hypertrophy complicated left heart failure.Methods：A total of 78 patients with hypertensive left ventricular hypertrophy complicated left heart failure（HF）were enrolled as HF group,induding class Ⅱ group（38 cases）and class Ⅲ group（40 cases）another 120 healthy subjects undergoing physical examination were selected as healthy control group.All subjects received UCG,and left ventricular end-diastolic diameter（LVEDd）,left atrial diameter（LAD）,left ventricular ejection fraction（LVEF）and ratio of mitral early diastolic peak flow velocity/mitral early diastolic maximal velocity（E/Ea）examination and were compared among all groups.Results：Compared with healthy control group,there were significant rise in LVEDd [（45.63±5.32）mm vs.（57.24±11.16）mm],LAD[（33.25±6.54）mm vs.（50.53±10.92）mm]and E/Ea [（8.01±4.21）vs.（14.21±3.52）],and significant reduction in LVEF [（69.73±11.43）% vs.（54.36±10.51）%]in observation group,P〈0.05 all.Compared with class Ⅱ group,there were significant rise in LVEDd [（57.12±9.53）mm vs.（66.20±7.35）mm],LAD [（46.36±6.94）mm vs.（53.14±7.21）mm]and E/Ea [（12.89±3.02）vs.（15.17±3.53）],and significant reduction in LVEF [（46.31±10.23）% vs.（38.75±8.61）%]in class Ⅲ group,P〈0.05or0.01.Conclusion：UCG possesses certain diagnostic value for hypertensive left ventricular hypertrophy complicated left heart failure,it can be used to assess disease severity in these patients,which is worth extending and application.
