摘要
目的探讨脓毒性休克患者血清presepsin与降钙素原(PCT)的相关性及对预后的影响研究。方法选取该院2013年1月—2015年11月收治的83例脓毒性休克患者,入住重症医学科后检测血清presepsin与PCT浓度,同时记录相关化验指标及患者基本情况,根据预后分为生存组及死亡组,比较二组presepsin与PCT的相关性,并先单因素后多因素分析对预后的影响。结果生存组及死亡组presepsin与PCT相关性分析,P<0.05有统计学意义,且r值均为0<r<1,具有正相关性。单因素分析5个因素(presepsin、PCT、CRP、APACHEII评分、乳酸浓度)差异有统计学意义(P<0.05),多因素Logistic分析血清presepsin浓度、APACHEII评分是脓毒性休克预后的独立危险因素(P<0.05),且OR>0。PCT、CRP、乳酸浓度不是预后的独立危险因素(P>0.05)。结论脓毒性休克患者presepsin与PCT具有正相关性,presepsin是预后的独立危险因素,对预后的评估价值优于PCT。
Objective To explore the correlation between presepsin and procalcitonin （PCT） in patients with septic shock and to study the impact on prognosis. Methods A total of 83 patients with septic shock admitted to autbor＇s hospital from January 2013 to November 2015 were enrollded. The serum concentration Of presepsin and PCT were detected when the patients get into the ICU. All the patients were recorded about their related laboratory index and the traditional risk factors. All patients had been divided into two groups （survival group and death group） according to the prognosis. Correlation was compared between presepsin and PCT in each group, The univariate factor analysis and logistic regression analysis of the related risk factors were performed on prognosis. Results There was a positive correlation between presepsin and PCT（P〈0. 05）, （0〈r〈1）, The univariate factor analysis indicated that 5 factors （presepsin, PCT, CRP, APACHEII score, the concentration of lactic acid） had statistical significance between the two groups（P〈0.05）. Logistic regression analysis showed that presepsin and APACHEII score were independent risk factors in patients with septic shock（P〈0.05）. PCT,CRP and the concentration of lactic acid were not independent risk factors for prognosis（P〉0.05）. Conclusions There is a positive correlation between presepsin and PCT in the patients with septic shock. Presepsin is an independent risk factor for prognosis. Presepsin is better to predict the prognosis than PCT.
