摘要 目的探讨细菌、真菌感染对血小板的影响,为临床感染性疾病诊断、病情评估、预防治疗提供试验依据。方法选择2011年7月-2015年12月医院收治的感染性疾病患者160例为试验组,其中细菌感染123例、真菌感染37例;另选择同期健康体检者100名为对照组,比较血小板参数,包括血小板数目、血小板平均体积(MPV)、血小板压积(PCT)、血小板体积分布宽度(PDW)四项的变化,从而探讨细菌、真菌感染对血小板的影响。结果试验组患者均出现血小板数量减低、MPV增大、PCT减低、PDW增大,与对照组比较,差异均有统计学意义(P<0.05),且革兰阴性杆菌、真菌感染血小板以上参数变化更明显;革兰阳性杆菌、革兰阳性球菌、革兰阴性杆菌、真菌在血小板数目、PCT、MPV、PDW的相互比较中,均差异有统计学意义(P<0.05);各试验组血小板数目与MPV均呈显著负相关。结论细菌、真菌感染会造成血小板数量减低、MPV增大、PCT减低、PDW增大,血小板计数简便易操作,能较正确、敏感地反映出感染状态,可借助血小板计数及相关参数检测来辅助判定感染类型,为临床感染性疾病的诊断及鉴别诊断提供有效参数。 OBJECTIVE To explore the influence of bacterial ,fungal infections on platelets so as to provide labora‐tory evidence for diagnosis of clinical infectious disease ,assessment of illness condition ,and prevention and treat‐ment .METHODS Totally 160 patients with infectious disease who were treated in the hospital from Jul 2011 to Dec 2015 were chosen as the experimental group ,including 123 cases of bacterial infection and 37 cases of fungal infec‐tion;meanwhile ,100 healthy people who received the physical examination were set as the control group .The changes of the platelet parameters ,including the platelet counts ,mean platelet volume (MPV) ,procalcitonin (PCT) ,and platelet distribution width (PDW) ,were observed and compared;the influence of the bacterial ,fun‐gal infection on the platelets was explored .RESULTS In the experimental group ,the platelet counts were de‐creased ,the MPV was enlarged ,the level of PCT was reduced ,and the PDW was enlarged ,as compared with the control group ,and the difference was significant (P〈0 .05) .The above platelet parameters of the patients with gram‐negative bacilli infection ,or fungal infection changed more significantly .There was significant difference in the platelet counts ,level of PCT ,MPV ,or PDW among the patients with gram‐positvie bacilli infection ,the pa‐tients with gram‐negative bacilli infection ,the patients with gram‐positive bacilli infection ,and the patients with fungal infection (P〈0 .05);the platelet count of the experimental group was negatively correlated with the MPV . CONCLUSION The bacterial ,fungal infection may lead to the decrease of the platelet counts ,enlargement of MPV ,reduction of level of PCT ,and enlargement of PDW .The platelet counting is simple and convenient and can reflect the state of infection in a accurate ,sensitive way .The platelet count in combination with detection of the related parameters can be used for auxiliary determination of types of infection and provide effective parameters for differential diagnosis of clinical infectious disease .
