摘要
目的:探讨血清胎球蛋白A( AHSG)及脂联素水平变化与代谢综合征( MS)的关系。方法采用随机数字表法抽取本院726名体检者为研究对象,收集相关临床资料,酶联免疫吸附法( ELISA)测定AHSG及脂联素水平。结果与非MS组相比,MS组血清AHSG水平升高,脂联素水平降低。在MS组及非MS组中AHSG与脂联素均呈负相关,尤其在MS组更为显著;多元线性回归分析显示,MS组中以AHSG为因变量,脂联素经校正相关因素后与AHSG呈独立负相关(β=-0.239, P〈0.01);以脂联素为因变量, AHSG亦与其呈独立负相关(β=-0.257, P〈0.01);在非MS组中,未观察到两者间的独立相关性。 Logistic回归分析显示,校正其他混杂因素后,MS的发生风险随着AHSG四分位水平的逐渐增加而增加,随着脂联素四分位水平的逐渐增加而降低(均P趋势〈0.01)。分层卡方检验显示,AHSG在高值组与脂联素在低值组的对应度及AHSG在低值组与脂联素在高值组的对应度在MS组(OR=3.443,P〈0.01)显著高于非MS组(OR=1.488,P=0.024);Mantel-Haenszel检验中,控制混杂因素MS时,AHSG与脂联素高值与低值的对应度亦明显高于两者中高值与高值或低值与低值的对应度(OR=1.791,P〈0.01)。结论 AHSG可能是MS的危险因子,脂联素是保护因子,两者在调节机体代谢活动中具有协同性, MS可能增强两者间的交互作用。
Objective To investigate the associations of serum levels of alpha2-Heremans-Schmid glycoprotein(AHSG) and adiponectin (APN) with metabolic syndrome (MS). Methods A total of 726 subjects were randomly selected from physical examination population using the random number table. The clinical data were collected and serum levels of AHSG and APN were measured by ELISA. Results Serum AHSG level was higher and APN level was lower in MS group compared to those in non-MS group. There were negative correlations between AHSG and APN in both MS and non-MS groups, especially more significant in MS group (P〈0. 01). Multivariate linear regression analysis showed that when AHSG was as the dependent variable, APN was associated independently and negatively with AHSG after adjustment for other related factors (β=-0. 239, P〈0. 01) in MS group, and vice versa (β=-0. 257, P〈0. 01), but the independent associations between them were not observed in non-MS group. The risk of MS was increased with increasing quartile of AHSG and decreased with that of APN in logistic regression model after adjustmentforotherconfoundingfactors(bothPfortrend〈0.01). Layeredchi-squaretestshowedthatthecorresponding degrees between higher AHSG and lower APN levels or between lower AHSG and higher APN levels were higher in MS group (OR=3. 443, P〈0. 01) compared to those in non-MS group (OR=1. 488, P=0. 024). Mantel-Haenszel test showed that the associations of AHSG in higher/lower range and APN in lower/higher range were also closer than those ofboth AHSG and APN in higher/lower ranges after adjusting the impact of MS (OR = 1. 791, P〈0. 01). Conclusions AHSG may be a risk factor for MS and APN as a protective factor. AHSG and APN have a synergistic effect on metabolism regulation while MS may increase their interaction.
