摘要
目的探讨益气活血汤联合尿激酶对结核性胸膜炎患者炎症因子、E-选择素、LDH水平影响。方法70例胸膜炎患者随机分为研究组与对照组，每组35例。对照组患者给予尿激酶治疗，研究组在尿激酶治疗基础上结合益气活血汤治疗。两组患者疗程均为8周。对比分析两组患者治疗总有效率、胸腔积液吸收时间和住院时间，治疗前和治疗8周血清炎症因子、E-选择素、LDH水平及药物副反应情况。结果研究组治疗总有效率明显高于对照组（91．43％vs．65．71％），差异有统计学意义（P〈0．05）；研究组胸腔积液吸收时间和住院时间均显著短于对照组（P〈0．05）；两组患者治疗后IL-6和IL-8水平较治疗前下降，差异有统计学意义（P〈0．05）；研究组患者治疗后IL-6和IL-8水平低于对照组（P〈0．05）；两组患者治疗后E-选择素、LDH水平较治疗前下降（P〈0．05）；研究组患者治疗后E-选择素、LDH水平低于对照组（P〈0．05）；两组患者在治疗期间均未出现严重药物副反应。结论益气活血汤联合尿激酶对结核性胸膜炎患者疗效明显，可降低炎症因子IL-6和IL-8、E-选择素、LDH，具有重要研究价值。
Objective To investigate the effect ofYiqi Huoxue decoction combined with urokinase on the levels of inflammatory factors, E-selection, and LDH in patients with tuberculous pleurisy. Methods 70 cases of tuberculous pleurisy were randomly divided into research group （n=35） and control group （n=35）. Control group was treated with urokinase, while research group was given Yiqi Huoxue decoction on the basis of the treatment in control group. Both groups were treated for 8 weeks. Made a comparison and analysis of the total effective rate, absorption time of pleural effusion, and the length of stay of two groups, the levels of inflammatory factors, E-selection, and LDH before and 8 weeks after treatment, and adverse drug reactions. Results The total effective rate in research group was significantly higher than that in control group （91.43% vs. 65.71%）, with statistically significant difference （P〈0.05）. The absorption time of pleural effusion and the length of stay in research group were significantly shorter than those in control group, with statistically significant differences （P〈0.05）. After treatment, IL-8 and IL-6 levels in both groups significantly decreased （P〈0.05）, IL-6 and IL-8 levels in research group were significantly lower than those in control group （P〈0.05）. After treatment, the levels of E-selection and LDH in both groups significantly decreased （P〈0.05）, the levels of E-selection and LDH in research group were significantly lower than those in control group （P〈0.05）. There were no serious adverse drug reactions during the treatment course in both two groups. Conclusion Yiqi Huoxue decoction combined with urokinase in the treatment of patients with tuberculous pleurisy has striking clinical effect, which can reduce the levels of inflammatory factors （IL-6 and IL-8）, E-selection, and LDH, with important research value.
