摘要
目的探讨内镜下粘膜剥离术(ESD)对大肠侧向发育性肿瘤(LST)的治疗价值。方法收集2009年1月--2015年9月在我院消化内镜中心治疗的LST33例,回顾性分析内镜表现及术前,术后病理及并发症。结果通过ESD完整切除病灶例29例(87.9%),术中出血1例(3.0%),迟发出血1例(3.0%),穿孔2例(6.0%),术后病理--非肿瘤及非浸润低级别上皮内瘤变10例(30.3%),非浸润高级别上皮内瘤变19例(57.6%其中高级别腺瘤16例非浸润粘膜内癌3例),浸润性癌4例(12.1%其中高分化腺癌3例,浸润超过粘膜下层的粘膜内癌1例)。结论内镜下粘膜剥离术是大肠侧向发育性肿瘤的首选治疗方法。
Objective To explore the therapeutic value of endoscopic mucosal dissection in the treatment of colorectal cancer.Methods LST33 cases hospitalized in digestive endoscopy center of our hospital from January 2009 to September 2015 were collected.Endoscopic results, preoperative pathology, postoperative pathology and all complications were analyzed retrospectively. Results Complete resection by ESD accounted for 29 cases（87.9%）. Hemorrhage during operation occurred in 1 cases（3%）. Delayed hemorrhage occurred in 1 cases（3%）. Perforation occurred in 2 cases（6%）. For postoperative pathology, there were 10 cases with low grade intraepithelial neoplasia（30.3%）. There were 19 cases with advanced non- invasive carcinoma（57.6%） among which there were 16 cases belonging to high grade adenocarcinoma and 3 cases being high-grade intramucosal carcinoma. Invasive carcinoma accounted for 4 cases（12.1%） among which 3 cases were high differentiated adenocarcinoma and 1 case was intramucosal carcinoma in the lower position of mucosa）. Conclusion Endoscopic mucosal dissection is the first choice in the treatment of colorectal cancer.
