Systematic evaluation on effect of psychological intervention for health related quality of life in patients with postoperative breast cancer

摘要 [目的]明确心理干预对乳腺癌术后病人健康相关生活质量（简称生活质量）的影响。[方法]利用计算机检索PubMed、Web of Science、LISTA、Library of Congress、Acad Search Prem、Cochrane、中国生物医学文献数据库、万方数据库、中国知网数据库和维普数据库,由两名研究者独立筛选文献、评价文献质量和提取数据,采用Revman5.3软件进行Meta分析。[结果]共纳入29项随机对照试验研究,2 649例病人,干预组1 342例,对照组1 307例,干预方法主要包括心理疏导、心理支持、认知干预和行为干预,干预时间以6周-8周为主,随访时间以3个月-6个月为主,生活质量评价工具主要为生活质量核心量表（QLQ-C30）、乳腺癌病人生命质量测定量表（FACT-B）以及生活质量综合评定问卷（GQOLI-74）。除FACT-B总分比较结果外,QLQ-C30总分（Z=4.60,P〈0.01）和GQOLI-74总分（Z=3.20,P〈0.01）比较均显示干预组病人生活质量高于对照组,差异有统计学意义（P〈0.05）。[结论]心理干预能够有效改善乳腺癌术后病人生活质量,但乳腺癌术后病人心理干预时间、频率等参数标准以及国内乳腺癌病人特异生活质量量表的开发应用有待进一步探讨。 Objective To clarify the influence of psychological intervention on health related quality of life （QOL） of patients with postoperative breast cancer. Methods： Two researchers independently screened and evaluated the literatures and extracted data by PubMed,Web of Science, LIST A , Library of Congress, Acad Search Prem, Cochrane, Chinese biomedical literature database, Wanfang database, CNKI database and VIP database, Meta analysis was carried out by Revman 5. 3 software. Results： A total of 29 randomized controlled trials, 2649 patients were included in the study, 1342 cases in intervention group, 1307 cases in control group. The intervention methods mainly included psychological counseling, psychological support, cognitive intervention and behavior intervention,intervention time for 6 to 8 weeks, followed up from 3 months to 6 months. Assessment tools of quality of life mainly were QLQ - C30,FACT - B and GQOLI - 74. In addition to FACT - B score comparison results, the total score of QLQ - C30 （Z =4. 60,P 〈0. 01） and GQOLI - 74 score （Z =3. 20 , P = 0. 001） comparison showed the quality of life of patients in intervention group was higher than that in control group,there was statistically significant difference （P 〈0. 05）. Conclusion： Psychological intervention could improve the quality of life of patients with postoperative breast cancer,but time and frequency of psychological intervention on postoperative breast cancer patients as well as the development and application of specific scale of quality of life to Chinese breast cancer patients need to be probed into further.
