摘要
目的 分析连云港地区孕妇孕中期筛查结果,确立筛查标志物(AFP、freeβ-h CG)的中位数,评价筛查软件适用性。方法 利用时间分辨免疫荧光分析法,对22 342例孕15~21+6周单胎妊娠孕妇检测血清中AFP、freeβ-h CG含量,通过Life Cycle 3.2软件进行风险计算;以Elipse统计分析模块拟合新中位数方程,比较新中位数与软件内嵌中位数,计算风险结果,比较筛查效率。结果 经比较发现,连云港地区中位数比系统内嵌中位数偏低7.56%~8.93%,存在统计学差异(P=0.000),连云港地区freeβ-h CG中位数比系统内嵌中位数偏高13.52%~35.77%,存在统计学差异(P=0.000)。AFP各孕周中位数偏差幅度不大。随体重增加,连云港地区体重中位数与系统内嵌中位数偏差变大。新中位数方程与内嵌中位数有统计学差异(P〈0.05),更换中位数后,筛查高风险率由7.27%降到4.88%,高风险率降低率为32.87%,筛查效率提高。结论 用数据分析软件拟合本实验室中位数方程可以提高筛查效率,此中位数方程对连云港地区产前筛查有指导意义。
Objective To analyze the screening results of serum markers among pregnant women during the second trimester of preg- nancy in Lianyungang region, establish medians of screening markers (alpha fetoprotein, AFP; free [3-human chorionic gonadotropin, free β-hCG) , evaluate the applicability of screening software. Methods Time-resolved immunefluorometry was used to detect the serum con- tents of AFP and free β-hCG in 22 342 pregnant women during 15-21.6 gestational weeks, LifeCycle 3. 2 software was used to evaluate the risk. Elipse statistical analysis module was used to build the new median equation, the new medians and embedded medians in the system were compared, the risk result was evaluated, and the screening efficiencies were compared. Results The median of AFP in Lianyungang region was lower than embedded medians in the system by 7.56% -8.93% , there was statistically significant difference (P= 0. 000) . The median of free β-hCG in Lianyungang region was higher than embedded medians in the system by 13.52% -35.77% , there was statistically significant difference (P = 0. 000) . The deviation amplitudes of medians during different gestational weeks were small. The deviation ampli- tude of weight median between Lianyungang region and the system largened. There was statistically significant difference between new medi- ans equation and embedded medians in the system ( P〈0. 05 ) . After changing median, the high risk rate of screening decreased from 7.27% to 4. 88%, the decreasing rate of high risk was 32. 87%, the screening efficacy increased. Conclusion Building the new median e- quation using data analyzing software can improve screening efficiency, which is instructive to prenatal screening in Lianyungang region.
