摘要
目的探讨与建立适合我国人群孕中期唐氏综合征筛查的血清标志物中位数倍数校正模型。方法利用非线性加权回归法构建血清学指标与孕龄的回归模型,根据PRESS统计量和校正决定系数选取最优模型校正血清学指标,利用累积和图法与国外常用的筛查软件Lifecycle 3.0、2T软件内置的各中位数进行比较。结果血清学指标与孕龄间有较好的回归关系,利用自建模型校正后,孕中期孕妇甲胎蛋白、游离人绒毛膜促性腺激素和游离雌三醇的累积和图在0附近,而利用Lifecycle 3.0、2T软件内置的中位数校正后,甲胎蛋白、游离人绒毛膜促性腺激素和游离雌三醇的累积和图远远偏离0。结论基于我国人群建立的孕中期孕妇血清标志物中位数倍数校正模型比国外筛查软件中内置的中位数模型更适合我国孕中期孕妇的产前筛查。
Objective To establish appropriate correction models of serum markers for prenatal screening of the patients with Down syndrome at the second trimester.Methods Nonlinear Weighted Regression models were employed to regress maternal serum markers and gestational age.The optimal models were selected by PRESS Statistics and R2adj.Then the medians were compared with those using such screening software as Lifecycle 3.0 and 2T by cumulative sum plots.Results The optimal models between maternal serum markers and gestational age were well built.The cumulative sum of log10 (MoM of AFP),log10 (MoM of free β-HCG) and log10 (MoM of uE3) for the models was near to zero,while those values for Lifecycle 3.0 and 2T were far deviated from zero.Conclusion Our correction model for maternal serum markers is more applicable than the models built by Lifecycle 3.0 and 2T for Chinese population at the second trimester screening.
